普米克令舒雾化吸入治疗危重新生儿气管插管导致的喉头水肿的研究 被引量:2

摘要 目的 观察分析普米克令舒雾化吸入治疗危重新生儿因气管插管导致的喉头水肿的效果。方法回顾性研究78例危重气管插管导致喉头水肿的新生儿,将其分为观察、对照两组,前者拔管后接受普米克令舒雾化吸入治疗;后者拔管拔管后不接受任何治疗。比对两组患儿喉头水肿症状持续时长,比对两组患儿拔管氧分压、二氧化碳分压、呼吸频率、心率四项指标水平。结果 观察组患儿喉头水肿症状持续时间为(15.23±3.69)h,低于对照组的(32.83±6.82)h,P<0.05。观察组患儿雾化吸入后氧分压为(75.45±6.28)mmHg、高于对照组的(64.17±5.86)mmHg;观察组二氧化碳分压、呼吸频率、心率水平为(42.41±4.28)mmHg、(46.33±3.86)次/min、(132.18±9.64)次/min,均低于对照组的(49.27±4.63)mmHg、(54.69±4.76)次/min、(145.21±10.78)次/min,P值均<0.05。结论普米克令舒雾化吸入治疗危重新生儿气管插管导致的喉头水肿时,能够显著缩短患儿水肿持续时长,改善包含氧分压在内的多项指标。 Objective To observe and analyze the effect of Pulmicort Respules aerosol inhalation in the treatment of laryngeal edema caused by endotracheal intubation in critically ill newborns.Methods 78 newborns with laryngeal edema caused by critical gas tube intubation were studied retrospectively.They were divided into observation and control groups.The former received Pulmicort Respules atomization inhalation after extubation;The latter did not receive any treatment after extubation.The duration of laryngeal edema symptoms was compared between the two groups,and the levels of oxygen partial pressure,carbon dioxide partial pressure,respiratory rate and heart rate were compared between the two groups.Results The duration of laryngeal edema in the observation group was (15.23±3.69) h,lower than that in the control group (32.83±6.82) h,P < 0.05.The oxygen partial pressure in the observation group was (75.45±6.28) mmHg,which was higher than that in the control group (64.17±5.86) mmHg;The levels of carbon dioxide partial pressure,respiratory rate and heart rate in the observation group were (42.41±4.28) mmHg,(46.33±3.86) times / min and (132.18±9.64) times / min,which were lower than those in the control group (49.27±4.63) mmHg,(54.69±4.76) times / min and (145.21±10.78) times / min,P < 0.05.Conclusion Pulmicort Respules aerosol inhalation can significantly shorten the duration of laryngeal edema caused by endotracheal intubation in critically ill newborns and improve many indexes including oxygen partial pressure.
