肠道菌群多样性变化与手足口病相关性的分析

摘要 目的探讨手足口病患儿肠道菌群的变异性和多样性,分析肠道菌群与手足口病的相关性.方法收集15例普通手足口病患儿(HD组)、22例重症手足口病患儿(HE组)和20例健康儿童(NC组)的粪便标本,应用16SrDNA高通量测序技术检测肠道菌群的变化.结果肠道菌群α多样性显示,NC组shannon指数高于HD组(P<0.05),而HD组与HE组,差异无统计学意义(P>0.05).单因素方差分析显示,NC组中双歧杆菌科的相对丰度降低,而瘤球菌科、梭状芽孢杆菌科的相对丰度升高,HD组中链球菌科相对丰度高于HE组(P<0.05).瘤球菌科、梭状芽孢杆菌科和链球菌科的相对丰度与发热天数、咽峡部疱疹、手足臀部疱疹、粪便病毒载量和血液病毒抗体阳性显著相关(P<0.05).瘤球菌科的相对丰度与惊跳显著相关(P<0.05),梭菌科相对丰度与呕吐、惊跳和肢体抖动显著相关(P<0.05).相关性分析显示,HD组和HE组的肠道菌群相似,NC组中肠道菌群与HE组和HD组不同.结论病毒感染改变肠道菌群结构,肠道菌群与手足口病的发生、发展可能存在相关性. Objective To explore the variability and diversity of intestinal bacterial community composition(BBC)in hand foot and mouth disease(HFMD),and to analyze the correlation between intestinal bacterial community composition and disease.Methods We collected 15 cases of common HFMD(HD),22 HFMD patients with encephalitis(HE)and 20 normal children's faeces(NC),applied 16SrDNA high-throughput sequencing technology to detect the BCC changes.Results Boxplots of intestinal bacterial alpha-diversity across groups showed that shannon index of NC was significantly higher than HE,however,the shannon index of HE had no significant difference with neither NC nor HE(P<0.05).One-way ANOVA revealed that the relative abundances of Bifidobacteriaceae was significantly lower in NC and the relative abundances of Ruminococcaceae and Clostridiaceae was significantly higher in NC,when the relative abundances of Streptococcaceae was significantly higher in HD than in HE(P<0.05).The relative abundances of Ruminococcaceae,Clostridiaceae and Streptococcaceae was significantly related to fever days,erythra in pharyngeal isthmus,erythra in hand foot and hip,viruses in faeces and viruses in blood.The relative abundances of Ruminococcaceae was significantly related to startle and the relative abundances of Clostridiaceae was significantly related to vomit,startle and limb jitter(P<0.05).The correlation analysis showed that the intestinal bacterial in the HD group and HE group were similar,and the intestinal bacterial in the NC group was different from that in the HE and HD groups.Conclusion This study suggests that virus infection changes the intestinal bacterial and there may be the relationship between intestinal bacterial and HFMD.
