黄芪片与缬沙坦胶囊联合应用在高血压并发微量蛋白尿治疗中的效果研究

Effect of combined application of Astragalus membranaceus tablet and valsartan capsule in patients with hypertension complicated with microalbuminuria

目的 探讨黄芪片联合缬沙坦胶囊在高血压并发微量蛋白尿患者中的应用及对血液流变学的影响。方法 选择2020年4月至2021年4月在上海市虹口区广中路街道社区卫生服务中心进行治疗的100例高血压并发微量蛋白尿患者,将患者使用数字表法随机分成对照组、观察组,每组患者50例。对照组予以常规西药治疗,观察组予以黄芪片联合缬沙坦胶囊进行治疗。比较治疗后2组患者的血压水平、肾功能水平、血液流变学变化情况及不良反应发生率。结果 治疗后观察组患者的收缩压、舒张压均低于对照组(P<0.05);治疗后2组患者的血肌酐、尿白蛋白/肌酐比值、血尿酸水平均低于治疗前(P<0.05),且观察组患者各水平均低于对照组(P<0.05);观察组经过治疗后全血高切黏度水平以及血浆黏度水平与对照组比较显著降低(P<0.05);观察组经过治疗后出现眩晕、低血压、肝肾功能异常、头痛及感染的不良反应总发生率10%(5/50),低于对照组24%(12/50)(P<0.05)。结论 黄芪片联合缬沙坦胶囊在高血压并发微量蛋白尿患者可以有效改善血压水平,提高肾功能,改善血液流变学变化水平,不良反应较少,值得推广应用。

Objective To investigate the application of Astragalus membranaceus tablet combined with valsartan capsule in patients with hypertension complicated with microalbuminuria and its influence on hemorheology.Methods One hundred patients with hypertension complicated with microalbuminuria admitted to Guangzhong Str eet Community Health Service Center of Shanghai Hongkou District from April 2020 to April 2021 were selected and randomly divided into control group and observation group using the number table method(n=50). The control group was treated with conventional western medicine, and the observation group was treated with Astragalus membranaceus tablet combined with valsartan capsule. Blood pressure, renal function, hemorheology, and the incidence of adverse reactions were compared between the two groups after treatment. Results After treatment, systolic blood pressure and diastolic blood pressure in the observation group were lower than those in the control group(P<0.05), and the levels of creatinine(Cr), urine albumin-to-creatinine ratio, and uric acid(UA) in the two groups were lower than before(P<0.05), and the levels of these indexes in the observation group were lower than that in the control group( P<0.05).After treatment, the whole blood high shear viscosity level and plasma viscosity level in the observation group were significantly decreased compared with the control group(P<0.05). After treatment, the total incidence of adverse reactions, such as vertigo, hypotension, abnormal liver and kidney function, headache, and infection in the observation group was(5/50, 10%), lower than that in the control group(12/50, 24%)( P<0.05). Conclusion Astragalus membranaceus combined with valsartan capsule can effectively improve blood pressure level, renal function, and hemorhe-ology change level in patients with hypertension complicated with microalbuminuria, with fewer adverse reactions,which is worthy of popularization and application.
