结石收集过滤器在硕通镜碎石手术中的应用 被引量:2

摘要 目的:探讨结石收集过滤器在硕通镜碎石手术中的应用价值。方法:选择2019年1月—2020年12月兴国县人民医院收治的行硕通镜碎石术治疗的36例泌尿系结石患者作为研究对象,按随机数表法分为观察组和对照组,每组各18例。两组患者均在全麻下行常规硕通镜手术,观察组术中使用自制结石过滤器收集结石。比较两组患者术中指标(包含术中出血量、手术用时)和术后相关指标(包含住院时间、结石收集率、术后24 h疼痛评分和术后1个月结石清除率、Ⅱ期碎石发生率)。结果:两组患者术中出血量相比,差异无统计学意义(t=0.140,P>0.05)。观察组手术用时为(30.21±6.12)min,短于对照组的(34.85±5.24)min,差异有统计学意义(t=2.443,P<0.05)。两组患者住院时间、术后24 h疼痛评分术后1个月结石清除率、Ⅱ期碎石发生率相比,差异无统计学意义(t=0.097、0.278,P>0.05)。观察组结石收集率为94.44%(17/18),高于对照组的77.78%(14/18),差异有统计学意义(P<0.05)。结论:硕通镜碎石手术中使用结石收集过滤器可缩短手术时长,提高结石收集率,有助于提高结石分析送检率。 Objective: To explore the application value of stone collection filter in Shuotong endoscope lithotripsy. Methods:Thirty-six patients with urinary calculi who underwent lithotripsy in the hospital from January 2019 to December 2020 were selected as the research subjects, and divided into an observation group and a control group according to the random number table method,with 18 cases in each group. Both groups of patients underwent conventional Shuotong endoscopy surgery under general anesthesia,while the observation group used a self-made stone filter to collect stones during the operation. The intraoperative indicators(including intraoperative blood loss and operation time) and postoperative related indicators(including hospitalization time, stone collection rate, 24-hour postoperative pain score, stone clearance rate at 1 month after operation, and incidence of stage II lithotripsy) were compared between the two groups. Results: There was no significant difference in intraoperative blood loss between the two groups(t=0.140, P>0.05). The operation time of the observation group was(30.21±6.12) min, which was shorter than(34.85±5.24) min of the control group, and the difference was statistically significant(t=2.443, P<0.05). There was no significant difference in hospitalization time, 24-hour postoperative pain score, stone clearance rate at 1 month after operation and incidence of stage Ⅱlithotripsy between the two groups(t=0.097, 0.278, P>0.05). The stone collection rate in the observation group was94.44%(17/18), which was higher than 77.78%(14/18) in the control group, and the difference was statistically significant(P<0.05). Conclusion: The use of a stone collection filter in Shuotong endoscopy lithotripsy can shorten the operation time, improve the stone collection rate, and help improve the rate of stone analysis and inspection.
