非布司他对合并高尿酸血症的维持性血液透析患者残余肾功能的保护效果分析

Analysis of the protective effect of Febuxostat on residual renal function in maintenance hemodialysis patients with hyperuricemia

目的:分析非布司他对合并高尿酸血症的维持性血液透析患者残余肾功能(RRF)的保护效果。方法:选取2018年9月—2019年9月南京市浦口医院收治的合并高尿酸血症的维持性血液透析患者60例,采用随机数字表法分为非布司他组和别嘌醇组各30例。于治疗前、治疗后24周分别记录并分析两组血尿酸水平和RRF变化,比较两组有效率。结果:经过24周的规范化治疗后,非布司他组患者的血尿酸低于别嘌醇组,而R R F和估算肾小球滤过率(e G F R)高于别嘌醇组,差异均有统计学意义(P<0.001)。非布司他治疗合并高尿酸血症的维持性血液透析患者总有效率为93.33%,高于别嘌醇组的60.00%,差异有统计学意义(P<0.05)。结论:相较于别嘌醇,给予合并高尿酸血症且维持血液透析患者非布司他治疗24周后,患者的血尿酸水平下降更显著,更有利于改善RRF,值得临床应用。

Objective To evaluate the protective effect of Febuxostat on residual renal function(RRF)in maintenance hemodialysis patients with hyperuricemia.Methods This study is a prospective study.60 maintenance hemodialysis patients with hyperuricemia treated in Nanjing Pukou Hospital from September 2018 to September 2019 were randomly divided into Febuxostat group and Allopurinol group with 30 cases in each group.The changes of serum uric acid and RRF were recorded and analyzed before and 24 weeks after treatment.Results After 24 weeks of standardized treatment,the serum uric acid of patients with Febuxostat was significantly lower than that of the Allopurinol group,while the RRF and estimated glomerular filtration rate(EGFR)were significantly higher than that of the Allopurinol group(P<0.001).The effective rate of Febuxostat in the treatment of maintenance hemodialysis patients with hyperuricemia was 93.30%,which was significantly higher than that of the Allopurinol group(60.00%),the difference was statistically significant(P<0.05).Conclusion Compared with Allopurinol,the level of serum uric acid in patients with hyperuricemia and maintenance hemodialysis decreased more significantly after 24 weeks of Febuxostat treatment,which was more conducive to improve RRF,and was worthy of clinical application.
