基于疼痛药学服务构建骨肿瘤术后幻肢痛服务模式

Construction of Phantom Limb Pain Care Mode After Bone Tumor Surgery Based on Pain Pharmaceutical Care

目的 基于疼痛药学服务探讨骨肿瘤术后幻肢痛服务模式的构建。方法 选取医院骨科骨肿瘤病区2018年1月至2021年12月拟行截肢术的患者67例。疼痛专职临床药师以骨肿瘤术后幻肢痛患者为中心,成立多学科疼痛服务团队和药剂科疼痛专项服务小组,并创建基于会诊的骨肿瘤拟截肢患者围术期疼痛服务模式和临床科室-临床药学部-调剂部门三方联动的疼痛服务模式。通过幻肢痛治疗药物前置审方,药物监测等精准药物服务体系,疼痛药物处方点评,以患者为中心的疼痛知识宣教及用药教育,为患者提供个体化用药服务。结果 患者的平均疼痛评分由干预前的6.4分降至干预后的2.4分,差异显著(P <0.05);满意度由干预前的70%升至干预后的88%,差异显著(P <0.05)。结论 骨肿瘤术后幻肢痛服务模式的构建可为疼痛专职临床药师开展规范化、全程化、同质化的药学服务工作提供参考,促进临床药学发展。

Objective To investigate the construction of phantom limb pain care mode after the bone tumor surgery based on thepain pharmaceutical care.Methods Sixty-seven patients who were to underwent the amputation in the bone tumor ward of theorthopedics department in the hospital from January 2018 to December 2021 were selected. Taking the patients with phantom limbpain after the bone tumor surgery as the center,the pain professional clinical pharmacists set up a multidisciplinary pain care team,a special pain care team in the pharmacy department,and constructed a consultation-based perioperative pain care mode for thepatients with bone tumor to be amputated and a pain care mode of the tripartite linkage of clinical department-clinical pharmacydepartment-dispensing department. Through the pre-prescription review of phantom limb pain drugs,precise drug care systemssuch as drug monitoring,pain drug prescription comments,pain knowledge education and medication education with patients as thecenter,individualized medication care were provided for patients.Results The average pain score of the patients decreasedsignificantly from 6. 4 points before the intervention to 2. 4 points after the intervention(P < 0. 05),while the satisfaction of thepatients increased significantly from 70% before the intervention to 88% after the intervention(P < 0. 05).Conclusion Theconstruction of phantom limb pain care mode after the bone tumor surgery can provide a reference for the pain professionalclinical pharmacists to carry out the standardized,whole-process and homogeneous pharmaceutical care,and promote thedevelopment of clinical pharmacy.
