中医定向透药结合耳穴压豆在全髋关节置换术后早期疼痛患者护理中的应用效果

Application eff ect of directional drug penetration combined with auricular acupoint pressure in nursing of patients with early pain after total hip arthroplasty

目的探讨中医定向透药结合耳穴压豆护理全髋关节置换术后早期疼痛的临床效果。方法选取2020年10月—2021年10月医院收治的全髋关节置换术后早期疼痛患者60例为研究对象,按基本资料匹配原则分为观察组和对照组,每组30例。对照组给予常规护理、镇痛处理及健康教育指导,观察组在对照组基础上给予中医定向透药结合耳穴压豆护理。观察比较术前1 d、术后第1、3天及术后1、2周的视觉模拟评分法(VAS)评分、肿胀值,术前1 d、术后随访3个月关节功能量表(WOMAC)评分。结果实施中医定向透药结合耳穴压豆护理后,VAS指标及肿胀值均随观察时间延长呈现降低趋势(P_(时间)<0.05),并且各时点均以观察组低于对照组(P_(组间)<0.05),随观察时间延长,组间差异逐渐增加(P_(交互)<0.05)。术后随访3个月,观察组的WOMAC量表评分低于对照组,差异有统计学意义(P<0.05)。结论中医定向透药结合耳穴压豆应用于全髋关节置换术后早期疼痛护理,可有效减少疼痛,改善肿胀症状,有利于关节功能恢复。

Objective To explore the clinical eff ect of directional drug penetration combined with auricular acupoint pressure in nursing early pain after total hip arthroplasty.Methods A total of 60 patients with early pain after total hip arthroplasty admitted to the hospital from October 2020 to October 2021 were selected as the research objects.According to the basic data matching principle,they were divided into an observation group and a control group,with 30 cases in each group.The control group was given routine nursing,analgesia treatment and health education guidance,and the observation group was given traditional Chinese medicine directional permeation combined with auricular acupoint pressure nursing on the basis of the control group.The Visual Analogue Scale(VAS)score,swelling value,the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score at 1 day before operation,1st and 3rd days after operation,1 and 2 weeks after operation were observed and compared.Results After the implementation of traditional Chinese medicine directional penetration combined with auricular acupoint pressure nursing,VAS score and swelling value showed a decreasing trend with the extension of observation time(P<0.05),at each time point,the observation group was lower than the control group(P<0.05),with the extension of observation time,the diff erence between groups gradually increased(P<0.05).After 3 months of follow-up,the WOMAC score of the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion The application of traditional Chinese medicine directional penetration combined with auricular acupoint pressure bean in early pain nursing after total hip arthroplasty could eff ectively reduce pain,improve swelling symptoms and conducive to the recovery of joint function.
