分析二甲双胍、口服氯雷他定片与滋阴除湿汤加减治疗糖尿病性老年皮肤瘙痒症阴虚湿恋证的效果

Analysis of the Effect of Metformin,Oral Loratadine Tablets and Modified Ziyin Chushi Decoction in the Treatment of Diabetic Senile Skin Pruritus with Yin Deficiency and Dampness Syndrome

目的 对糖尿病性老年皮肤瘙痒症患者使用二甲双胍、口服氯雷他定片和滋阴除湿汤进行治疗的效果做出分析。方法 对2020年6月—2021年12月期间前来济宁市兖州区中医医院治疗的68例糖尿病性老年皮肤瘙痒症患者进行研究,根据随机抽样法把患者随机分成常规组(34例)和研究组(34例)。常规组患者使用二甲双胍联合口服氯雷他定片进行治疗,研究组使用二甲双胍联合滋阴除湿汤加减进行治疗,观察患者治疗效果(瘙痒情况、血糖水平、治疗有效率)。结果 研究组治疗有效率为88.24%,高于常规组,差异有统计学意义(χ^(2)=4.191,P=0.040)。研究组血糖水平中,空腹血糖、餐后2 h血糖优于常规组,但差异无统计学意义(t=0.370、0.755,P>0.05),糖化血红蛋白(7.05±0.01)%对比常规组,差异有统计学意义(P<0.05)。研究组瘙痒评分中,瘙痒程度、瘙痒持续时间、瘙痒部位、瘙痒对睡眠影响评分分别为(1.71±0.16)分、(1.51±0.10)分、(1.31±0.11)分、(1.66±0.20)分,均低于常规组,差异有统计学意义(t=6.482、7.672、13.165、6.335,P<0.001)。结论 为糖尿病性老年皮肤瘙痒症阴虚湿恋证患者使用二甲双胍联合滋阴除湿汤进行治疗,可以改善患者瘙痒症状,且对血糖没有明显影响,临床应用价值较高。

Objective To analyze the effect of metformin,oral loratadine tablets and Ziyin Chushi decoction in the treatment of diabetic elderly patients with pruritus.Methods A study was conducted on 68 diabetic elderly patients with pruritus who came to the Yanzhou District Traditional Chinese Medicine Hospital of Jining City from June 2020 to December 2021.According to the random sampling method,the patients were randomly divided into a routine group(34 cases)and a study group(34 cases).The patients in the routine group were treated with metformin combined with oral loratadine tablets,and the study group was treated with metformin combined with Ziyin Chushi Decoction,the therapeutic effect of the patients(pruritus,blood sugar level,and effective rate of treatment) were observed.Results The effective rate of the study group was 88.24%,which was higher than that of the routine group,and the difference was statistically significant(χ^(2)=4.191,P=0.040).Among the blood glucose levels in the study group,fasting blood glucose and 2 h postprandial blood glucose were better than those in the routine group,but the difference was not statistically significant(t=0.370,0.755,P>0.05),and the glycated hemoglobin(7.05±0.01)% compared with the routine group,the difference was statistically significant(P<0.05).In the pruritus score of the study group,the pruritus degree,pruritus duration,itching site,and pruritus impact on sleep scores were(1.71±0.16)points,(1.51±0.10)points,(1.31±0.11)points,and(1.66±0.20)points,respectively,were lower than the conventional group,the difference was statistically significant(t=6.482,7.672,13.165,6.335,P<0.001).Conclusion The use of metformin combined with Ziyin Chushi Decoction for the treatment of diabetic elderly patients with pruritus due to yin deficiency and dampness can improve the itching symptoms of the patients.And there is no obvious effect on blood glucose,and the clinical application value is high.
