对比CT、MRI、X线在成人足舟骨坏死病中的影像学表现特征及确诊率

摘要 目的:探讨磁共振成像(MRI)、电子计算机断层扫描(CT)、X线在成人足舟骨坏死病中的影像学表现特征及确诊率比较。方法:选取我院2017年6月~2020年6月收治成人足舟骨坏死病患者36例,分别实施MRI、CT和X线检测,分析三种检测方式的影像学表现特征与检测确诊率。结果:36例成人足舟骨坏死病患者中,MRI影像均表现为骨髓水肿与足舟骨压缩变扁,CT影像可见足舟骨质密度增加与骨压缩变窄,而X线片表现为足舟骨骨质密度增加与足舟骨外侧部分变窄。MRI检查成人足舟骨坏死病的确诊率97.22%高于CT、X线检查83.33%、72.22%,且CT检查的确诊率83.33%高于X线检查72.22%,数据差异明显(P<0.05)。结论:MRI诊断对于成人足舟骨坏死病比CT、X线的确诊率更高,可作为成人足舟骨坏死病确诊的方法。临床可联合应用三种诊断方法,在X线初步诊断疾病的基础上,利用MRI、CT判断病变程度,以指导后续治疗。 Objective:To investigate the imaging characteristics and diagnosis rate of magnetic resonance imaging(MRI),computed tomography(CT)and X-ray in adult navicular osteonecrosis.Methods:From June 2017 to June 2020,36 adult patients with osteonecrosis of the navicular in our hospital were selected,and magnetic resonance imaging,computed tomography and X-ray examinations were carried out respectively,and the imaging features and the diagnosis rate of the three detection methods were analyzed.Results:Among the 36 adult patients with osteonecrosis of the navicular,magnetic resonance imaging images showed bone marrow edema and navicular bone compression and flattening,and computed tomography images showed navicular bone density.The increase is associated with narrowing of bone compression,while the radiograph shows increased bone density of the navicular bone and narrowing of the lateral part of the navicular bone.The diagnosis rate of adult navicular osteonecrosis by MRI examination was 97.22%higher than that of CT and X-ray examination,83.33%and 72.22%,and the diagnosis rate of CT examination was 83.33%higher than that of X-ray examination,which was 72.22%.The difference was significant(P<0.05).Conclusion:MRI diagnosis of adult navicular osteonecrosis has a higher diagnostic rate than computed tomography and X-ray,and can be used as a method for the diagnosis of adult navicular osteonecrosis.The three diagnostic methods can be combined in clinical practice.On the basis of the initial diagnosis of the disease by X-ray,MRI and CT can be used to judge the degree of the lesion to guide the follow-up treatment.
