门冬胰岛素联合地特胰岛素对妊娠期糖尿病患者血糖控制效果及母婴结局的影响

Effects of Insulin Aspart Combined with Insulin Detemir on Blood Glucose Control and Maternal and Infant Outcomes in Patients with Gestational Diabetes Mellitus

目的:探讨门冬胰岛素联合地特胰岛素对妊娠期糖尿病(GDM)患者血糖控制效果及母婴结局的影响。方法:选择2019年8月至2021年7月于赣州市妇幼保健院治疗的86例GDM患者,按随机数字表法将其分为两组,各43例。对照组采用门冬胰岛素治疗,观察组采用门冬胰岛素、地特胰岛素治疗。比较两组空腹血糖(FPG)、餐后2 h血糖(2 h PG)达标时间、血糖控制效果及母婴结局。结果:治疗后,观察组FPG达标时间(7.11±1.97)d、2 h PG达标时间(6.38±1.45)d短于对照组;观察组FPG(5.59±1.02)mmol/L、2 h PG(6.68±0.94)mmol/L及糖化血红蛋白(HbA1c)(5.97±1.22)%低于对照组,差异有统计学意义(P<0.05);两组剖宫产率、早产率及新生儿低血糖、新生儿低体重、巨大儿发生率比较,差异无统计学意义(P>0.05)。结论:GDM患者采用门冬胰岛素、地特胰岛素联合治疗是安全可行的,可降低血糖水平,提高血糖控制效果,缩短FPG、2 h PG达标时间,且对母婴结局影响小。

Objective:To investigate the effect of insulin aspart combined with insulin detemir on blood glucose control and maternal and infant outcomes in patients with gestational diabetes mellitus (GDM).Methods:A total of 86 patients with GDM who were treated in Ganzhou Maternal and Child Health Hospital from August 2019 to July 2021 were selected and divided into two groups according to the random number table method,with 43 cases in each group.The control group was treated with insulin aspart,and the observation group was treated with insulin aspart and insulin detemir.The fasting blood glucose (FPG),2 h postprandial blood glucose (2 h PG) reaching the target time,blood glucose control effect and maternal and infant outcomes were compared between the two groups.Results:The time to reach the standard of FPG in the observation group was (7.11±1.97) d,and the time to reach the standard of 2 h PG was (6.38±1.45) d,which was shorter than that of the control group (P0.05).Conclusion:The combined treatment of GDM patients with insulin aspart and insulin detemir is safe and feasible,which can reduce blood sugar level,improve blood sugar control effect,shorten the time to reach the standard of FPG and 2 h PG,and have little effect on maternal and infant outcomes.
