柴芍郁欢汤联合开郁针刺法治疗更年期抑郁症的疗效

Treatment of menopausal depression with Chaishao Yuhuan Decoction combined with Kaiyu Acupuncture

目的探讨柴芍郁欢汤联合开郁针刺法治疗更年期抑郁症的临床效果。方法选取我院2020年4月至2021年7月更年期抑郁症患者150例,根据随机数字表法分三组,各50例。对照组采用氟哌噻吨美利曲辛片治疗,观察1组在口服氟哌噻吨美利曲辛片的同时应用开郁针刺法治疗,观察2组在应用开郁针刺法基础上加用柴芍郁欢汤治疗。三组均治疗8周。治疗8周后统计三组患者治疗效果、不良反应发生率。治疗前、治疗8周比较三组汉密尔顿焦虑自评量表(HAMA)、汉密尔顿抑郁自评量表(HAMD)、更年期临床症状调查(Kupperman)、匹兹堡睡眠质量指数量表(PSQI)评分及黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、miR-155、miR-146a水平。结果观察组2组治疗总有效率高于对照组(P<0.05);治疗8周观察2组各量表评分、LH、FSH、miR-155、miR-146a低于观察1组、对照组,E2高于观察1组、对照组,同时观察1组上述指标改善优于对照组(P<0.05);三组不良发生率比较,差异无统计学意义(P>0.05)。结论柴芍郁欢汤联合开郁针刺法可有效减轻更年期抑郁症患者症状,调节性激素及miR-155、miR-146a表达,安全有效。

Objective To explore the clinical effect of Chaishao Yuhuan Decoction combined with Kaiyu acupuncture in the treatment of menopausal depression.Methods A total of 150 patients with menopausal depression in our hospital from April 2020 to July 2021 were selected and divided into 3 groups according to random number table method,with 50 cases in each group.The control group was treated with flupentixol and melitracen.Observation group 1 was treated with Kaiyu acupuncture while taking flupentixol and melitracen.Observation group2 was treated with Chaishao Yuhuan Decoction combined with Kaiyu acupuncture.All 3 groups were treated for 8weeks.The treatment effect and the incidence of adverse reactions in the 3 groups were observed.Hamilton Selfrating Anxiety Scale(HAMA),Hamilton Self-rating Depression Scale(HAMD),Menopause Clinical Symptom Survey(Kupperman),and Pittsburgh Sleep Quality Index(PSQI)were used to assess the treatment effect and serum Luteinizing Hormone(LH),Follicle Stimulating Hormone(FSH),Estradiol(E2),miR-155 and miR-146a levels were compared among the three groups.Results The total effective rate in the observation groups was higher than that in the control group(P<0.05).After 8 weeks of treatment,the score of each scale,LH,FSH,miR-155 and miR-146a in observation group 2 were lower than those in observation group 1 and control group,but E2 was higher.At the same time,the improvement of the above indexes in observation group 1 were better than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse effects among the three groups(P>0.05).Conclusion Chaishao Yuhuan Decoction combined with Kaiyu acupuncture is safe and effective for relieving the symptoms of menopausal depression,and regulating the expression of miR-155 and miR-146a.
