MRI测量肩峰角在肩峰下撞击综合征中的意义 被引量:1

摘要 目的:分析与探讨肩峰角与肩峰下撞击综合征的关系。方法:回顾分析2020年8月—2022年2月聊城市人民医院收治的57例因肩关节痛就诊并经临床和关节镜证实为肩峰下撞击综合征(subacromial impingement syndrome,SIS)的患者,另选取60名肩关节正常志愿者,均行MRI检查,由两名主治以上影像医师共同测量肩峰角,并观察有无肩峰骨赘形成及具体部位,并对肩峰角做ROC曲线得其最佳临界值及敏感度与特异度。结果:肩峰角大小对SIS诊断有统计学意义,当肩峰角大于7.1°,诊断为SIS的敏感度及特异度分别为82.5%、83.3%。肩峰骨赘多发生在肩峰外下缘。结论:从先天和后天两种因素探讨肩峰角过大产生SIS的机制,得到肩峰角与SIS关系密切。 Objective The aim was to investigate the association between acromion angulation(AA)with MRI and the subacromial impingement syndrome(SIS).Methods A retrospective analysis was made on 57 patients with shoulder pain and SIS confirmed by clinical and arthroscopy in Liaocheng People's Hospital from August 2020 to February 2022.Another 60 normal shoulder volunteers were selected for MRI examination.Two imaging physicians above the attending level jointly measured the acromion angle,observed the formation and specific location of acromion osteophytes,and performed ROC curve on the acromion angle to obtain the best critical value,sensitivity and specificity.Results In the correction of AA,there was significant statistical differences between SIS and control group.AA≥7.1°,the sensibility and specificity were 82.5%and 83.3%,respectively.The location of acromion spurs were mostly in the lateral lower surface.Conclusion By the analysis of the mechanism of congenital and acquired factors separately,there is a close association between AA and SIS.
