多焦点硬性角膜接触镜治疗高度近视的临床效果

摘要 目的观察不同年龄阶段青少年高度近视患者配戴多焦点硬性角膜接触镜(MFRGP)近视控制疗效。方法回顾性选取新乡医学院第三附属医院眼科2020年3月至2021年6月配戴多焦点RGP的青少年近视患者60例为MFRGP组,匹配相近样本量、同时间相同入组标准配戴单焦点框架眼镜的近视患者60例为单光框架眼镜(SV)组,等效球镜度为-6.00~-10.00 D,比较两组患者戴镜前后屈光度、眼轴长度变化及主觉视觉生活质量评分。结果MFRGP组屈光度变化小于SV组,8~13岁年龄段研究对象MFRGP组较SV组眼轴长度增长缓慢,差异有统计学意义(P<0.05)。14~18岁年龄段研究对象SV组与MFRGP组眼轴长度增长比较,差异无统计学意义(P>0.05)。两组视觉清晰度、对矫正视力的满意程度、矫正视力、远视力、近视力、昼夜视力波动及对矫正方式的依赖程度比较,差异无统计学意义(P>0.05)。与SV组相比,MFRGP组眩光、舒适度评分下降,差异有统计学意义(P<0.05)。结论配戴MFRGP可控制高度近视进展、延缓8~13岁青少年眼轴长度增长,效果显著。 Objective To observe the efficacy of myopia control with multifocal rigid gas permeable(MFRGP)contact lenses in adolescents with high myopia at different ages.Methods A total of 60 myopic children and adolescents wearing MFRGP contact lenses at the Department of Ophthalmology of the Third Affiliated Hospital of Xinxiang Medical University from March 2020 to June 2021 were selected as the MFRGP group,and another 60 myopic patients wearing single vision(SV)lenses were selected as the SV group by matching similar sample size and with the same inclusion criteria during the same period.All subjects were retrospectively analyzed.The degree of equivalent sphere lenses was-6.00 to-10.00 D.The c hanges of refractive error and oculi axes,as well as subjective vision-related quality of life s core before and after wearing glas ses were compared between the two groups.Results The change of refractive error in the MFRGP group was less than that in the SV group,and the growth of the oculi axes in the MFRGP group was slower than that in the SV group at the age of 8-13 years,with statistically significant differences(P<0.05).There was no statistically significant difference between the SV group and the MFRGP group in the growth of the oculi axes at the age of 14-18 years(P>0.05).There were no significant differences between the two groups of subjects in the scores of visual clarity,satisfaction with corrected visual acuity,corrected visual acuity,distant visual acuity,near visual acuity,day-light vision fluctuation and dependence on correction method(P>0.05).Compared with the SV group,glare and comfort scores decreased in the MFRGP group,with statistically significant differences(P<0.05).Conclusion Wearing MFRGP contact lenses can control the myopic progression of high myopia and significantly delay the growth of the oculi axes in adolescents aged 8-13 years.
