基于专科护理工作室对癌性伤口患者管理路径的探索与实践

Exploration and practice of management pathway for patients with Malignant Fungating Wounds based on specialized nursing studio

目的 :探索癌性伤口患者的管理路径,并评价其临床实践效果。方法 :将专科护理工作室与临床科室进行联合,形成多学科协作癌性伤口专项护理小组;通过小组讨论、动态评估,科学制定系统性、精准化的护理方案;同时拓展延续护理服务,开展了线上互动,授课、义诊等活动。结果 :2021年工作室护理癌性伤口患者11例,拟定癌性伤口个性化护理方案11套,开展社区义诊4次,社区义务授课4次,患者及家属满意度达95%以上。结论 :基于专科护理工作室的多学科协作式管理路径的实践为癌性伤口患者的高质量护理提供了保障,促进了癌性伤口的护理专业化发展,值得临床借鉴与应用。

Objective:To explore the management pathway for patients with Malignant Fungating Wounds(MFWs) based on specialized nursing studio and evaluate its clinical practical effects.Methods:The specialized nursing studio and clinical departments were collaborated to form a specialized multidisciplinary team for patients with MFWs.Through group discussion and dynamic evaluation,systematic and precise nursing plan were developed.Nursing services were expanded including online interaction,teaching,gratuitous treatment and other activities.Results:A total of 11 patients with MFWs were cared for in the studio,11 sets of individualized care plans for patients with MFWs were formulated,4 free community clinics and 4 community voluntary classes were conducted.The satisfaction of patients and their families was over 95%.Conclusion:The practice of multidisciplinary team based on specialized nursing studio provides dynamic and precise care services for patients with MFWs,and promotes the professional development of MFWs nursing,which is worthy of clinical reference and application.
