eSTIC技术联合Tei指数评价妊娠期高血压状态下胎儿右心室功能 被引量:1

CLINICAL VALUE OF ELECTRONIC SPATIOTEMPORAL IMAGE CORRELATION TECHNIQUE COMBINED WITH TEI INDEX IN EVALUATING FETAL RIGHT VENTRICULAR FUNCTION IN THE STATE OF GESTATIONAL HYPERTENSION

摘要 目的探讨使用电子矩阵时间-空间相关成像(eSTIC)技术中的虚拟器官计算机辅助分析(VOCAL)功能联合Tei指数评价孕妇妊娠期高血压状态时胎儿右心室功能的临床价值。方法选取在菏泽医学专科学校附属医院接受孕期检查的妊娠期高血压疾病孕妇105例,其中妊娠高血压45例、轻度子痫前期30例、重度子痫前期30例,选取75例正常孕妇作为对照组。应用eSTIC技术中的VOCAL功能获取胎儿右心室的舒张末期容积(EDV)和收缩末期容积(ESV),计算每搏输出量(SV)、心排血量(CO)和射血分数(EF);应用频谱多普勒检测胎儿右心室的等容时间、射血时间,计算Tei指数。结果对照组、妊娠高血压组和轻度子痫前期组间比较,右心室的等容时间、射血时间、Tei指数、EDV、ESV、SV、CO、EF差异均无统计学意义(P>0.05);重度子痫前期组与对照组相比,右心室的等容时间、Tei指数、EDV和ESV增大,右心室的射血时间、SV、CO和EF减小,差异均有统计学意义(F=2.014~6.931,P<0.05)。结论eSTIC技术的VOCAL功能联合Tei指数可以反映重度子痫前期状态下胎儿右心室功能,有助于临床及早发现异常、尽早采取治疗措施。 Objective To investigate the clinical value of virtual organ computer-aided analysis(VOCAL)function in electronic spatiotemporal image correlation(eSTIC)technique combined with Tei index in evaluating fetal right ventricular function in the state of gestational hypertension.Methods A total of 105 pregnant women with hypertensive disorders in pregnancy who underwent pregnancy examinations in The Affiliated Hospital of Heze Medical College were enrolled,among whom there were 45 women with gestational hypertension,30 with mild preeclampsia,and 30 with severe preeclampsia,and 75 normal pregnant women were enrolled as control group.VOCAL function in eSTIC technique was used to measure right ventricular end-diastolic volume(EDV)and end-systolic volume(ESV)of the fetus to calculate stroke volume(SV),cardiac output(CO),and ejection fraction(EF),and spectral Doppler was used to measure the isovolumic time and ejection time of the right ventricle to calculate Tei index.Results There were no significant differences in right ventricular isovolumic time,ejection time,Tei index,EDV,ESV,SV,CO,and EF between the control group,the gestational hypertension group,and the mild preeclampsia group(P>0.05),and compared with the control group,the severe preeclampsia group had significant increases in right ventricular isovolumic time,Tei index,EDV,and ESV and significant reductions in right ventricular ejection time,SV,CO,and EF(F=2.014-6.931,P<0.05).Conclusion The VOCAL function of eSTIC technique combined with Tei index can reflect fetal right ventricular function in severe preeclampsia,which helps with early detection of abnormalities and early treatment in clinical practice.
