社区血液透析降低患者治疗成本的研究

Study on Reducing the Cost of Treatment of Patients with Community Hemodialysis

目的探讨社区卫生服务中心维持性血液透析(maintenance hemodialysis,MHD)对降低患者治疗成本方面的作用。方法选取北京市昌平东小口社区卫生服务中心和北京市昌平区中医医院2018年7月—2019年6月80例MHD患者的临床资料进行回顾性分析,比较两医疗机构MHD患者一般资料、临床资料、生活质量、治疗成本情况。结果两医疗机构MHD患者一般资料均差异无统计学意义(P>0.05),社区MHD患者KDQOL-SFTM1.3量表评分、治疗成本分别为(4655.21±992.12)分、(123394.90±29480.84)元,三级医院MHD患者KDQOL-SFTM1.3量表评分、治疗成本分别为(4196.57±890.04)分、(168511.17±23084.08)元,差异有统计学意义(t=2.137、-7.617,P<0.05)。结论社区卫生服务中心维持性血液透析可以改善生活质量同时降低治疗成本,为社区开展维持性血液透析提供更多依据。

Objective To explore the effect of maintenance hemodialysis(MHD)in community health service center on reducing the cost of treatment for patients.Methods From July 2018 to June 2019,Clinical information of 80 MHD patients in Dongxiaokou Community Health Service Center of Changping District in Beijing and Changping District Hosital of Traditional Chinese Medicine were selected for retrospective analysis,and the general data,clinical data,quality of life and treatment costs of MHD patients in the two medical institutions were compared.Results There was no statistical difference in the general data of MHD patients in the two medical institutions(P>0.05).The KDQOL-SFTM1.3 scale score and treatment cost of MHD patients in the community were(4655.21±992.12)points and(123394.90±29480.84)yuan,respectively.The MHD patients in tertiary hospitals had KDQOL-SFTM1.3 scale scores and treatment costs were(4196.57±890.04)points and(168511.17±23084.08)yuan respectively,and the differences were statistically significant(t=2.137,-7.617,P<0.05).Conclusion Maintenance hemodialysis in community health service centers can improve the quality of life and reduce the cost of treatment,provide more evidence for community-based maintenance hemodialysis.
