我院门、急诊重点监控药品处方点评案例分析

Analyses on the effects of prescription review of key monitored drugs in clinical and emergency treatment departments of our hospital

目的了解北京大学第一医院密云院区门、急诊重点监控药品临床用药的现状,提升药师处方审核及点评的能力,促进该院重点监控药品的管理,为进一步提高临床合理使用重点监控药品提供技术支持。方法通过该院HIS系统,提取2020年该院重点监控药品处方,按月分别对门、急诊重点监控药品进行随机抽样点评,对点评出的不合理用药处方存在的问题进行评价分析。结果该院门、急诊涉及的重点监控药品存在不合理用药情况;不合理问题类型主要是诊断不全面、适应证不适宜、药品用法用量不适宜、联合用药不适宜及无适应证用药、超医保适应证用药等。结论该院重点监控药品存在一定程度的不合理用药现象,医院需进一步加强重点监控药品合理使用的管理,通过处方审核与点评,对不合理处方存在的问题采取及时反馈、公示等干预措施,促进临床安全用药、合理用药。

Objective To understand the current situation of clinical use of key monitored drugs in clinical and emergency departments of our hospital,inprove the ability of pharmacists to review and ccranent on prescriptions,promote the management of key monitored drugs,and provide technical support for farther improving the rational use of key monitored drugs in clinic.Methods Through the HIS system of our hospital,the prescriptions of key monitored drugs in 2020 were extracted,and the key monitored drugs in clinical and emergency departments were randomly sampled and evaluated on a monthly basis,and the problems of the irrational drug use prescriptions were evaluated and analyzed Results Irrational drug use was found in the key monitored drugs involved in clinical and emergency departments of our hospital;the main types of irrational problems are incomplete diagnosis,inappropriate indications,inappropriate oirug usage and dosage,inappropriate combination medication,medication without indications,medication beyond medical insurance indications,etc.Conclusion There is a certain degree of irrational drug use in the key monitored drugs in our hospital.The hospital needs to further strengthen the management of the rational use of key monitored oirugs,and take timely feedback,publicity and other intervention measures for the problems of irrational prescriptions through prescription review and raiment,so as to promote clinical safety and rational drug use.
