硬膜外麻醉与腰-硬联合麻醉在二次剖宫产术中的效果对比

Comparison of epidural anesthesia and combined spinal-epidural anesthesia in secondary cesarean section

目的分析硬膜外麻醉与腰-硬联合麻醉方法在二次剖宫产术中的效果。方法选取53例二次剖宫产产妇,根据产妇麻醉方式将产妇分为研究组(27例,实施腰-硬联合麻醉)与对照组(26例,实施硬膜外麻醉)。对比两组产妇麻醉效果、不良反应发生情况、平均住院时间、平均住院费用、初次母乳喂养时间、新生儿Apgar评分及平均出血量对。结果对照组并发症发生率为7.41%,研究组并发症发生率为11.54%,对比差异无统计学意义(P>0.05)。研究组运动神经阻滞程度优于对照组,麻醉起效时间快于对照组,差异有统计学意义(P<0.05)。研究组平均住院时间、初次母乳喂养时间短于对照组,平均出血量、平均住院费用少于对照组,新生儿Apgar评分高于对照组,差异有统计学意义(P<0.05)。结论对二次剖宫产产妇采用腰-硬联合麻醉,更能提高麻醉手术效果,可缩短产妇住院时间、初次母乳喂养时间,并减少并发症发生。

Objective To analyze the effect of epidural anesthesia and combined lumbar and epidural anesthesia in second cesarean section.Methods 53 parturients of second cesarean section were selected and divided into study group(27 cases,combined lumbar and epidural anesthesia)and control group(26 cases,epidural anesthesia)according to the anesthesia method.The anesthetic effect,adverse reactions,average hospitalization time,average hospitalization cost,initial breastfeeding time,neonatal Apgar score and average blood loss were compared between the two groups.Results The complication rate of the control group was 7.41%,and the complication rate of the study group was 11.54%,and the difference was not statistically significant(P>0.05).The degree of motor nerve block in the study group was better than that in the control group,and the onset time of anesthesia was faster than that in the control group,and the differences were statistically significant(P<0.05).The average length of hospital stay and first breastfeeding time in the study group were shorter than those in the control group,the average blood loss and average hospitalization cost were less than those in the control group,and the neonatal Apgar score was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Combined spinal-epidural anesthesia for second cesarean section can improve the anesthesia effect,shorten the hospitalization time and initial breastfeeding time,and reduce the incidence of complications.
