我国基层医疗服务居民获得感与首诊状况调查

Investigation on Residents’Sense of Gain in Grassroots Medical Services and the Status of the First-visit at Grassroots Medical Institution in China

目的:分析我国基层医疗服务居民获得感、首诊现状,为提升基层医疗服务居民获得感和落实基层首诊制度提供政策依据。方法:基于2019年全国51个抽样城市的7250份调查数据,运用描述性统计、单因素方差分析进行实证分析。结果:中国居民获得感水平得分为3.769,且呈现城镇高于农村,城市级别越高获得感水平越低,华东和中南地区较高、西南和西北居中、华北和东北地较低的现象。基层首诊状况得分为2.394,且呈现出农村高于城镇,普通地级市高于直辖/一线城市和副省级城市,华东、西北较高,华北、西南、中南地区居中而东北地区较低的现象。结论:我国基层医疗服务居民获得感和首诊状况均有待提升,建议做好提高基层医疗服务区域和项目供给均衡性,提升城乡基层医疗服务质量感,强化城乡居民基层首诊引导工作。

Objective:To analyze the residents’sense of gain in grassroots medical services and the status of the first-visit at grassroots medical institutions,so as to provide a policy basis for improving the residents’sense of gain and implementing the first-visit at grassroots medical institution system in China.Methods:Based on 7250 survey data from 51 sample cities in 2019,the empirical analysis was carried out by using descriptive statistics and one-way ANOVA.Results:The score of national residents’sense of acquisition level was 3.769,and show the following characteristics.Firstly,urban areas were higher than rural areas.Secondly,the higher the city level,the lower the sense of gain.Thirdly,East and Central South were higher,Southwest and Northwest were in the middle,and North and Northeast were lower.The score of the first-visit at grassroots medical institution status was 2.394,and showed the following characteristics.Firstly,rural areas were higher than urban areas.Secondly,ordinary prefecture level cities were higher than those municipality/first-tier cities and sub-provincial cities.Thirdly,East and Northwest were higher,North,Southwest and Central South were in the middle,while Northeast was lower.Conclusions:The sense of gain and the status of the first-visit at grassroots medical institution need to be improved.It is suggested to improve the balance of regional and project supply of grassroots medical services,improve the sense of quality in grassroots medical services in urban and rural areas,and strengthen the guidance of the first-visit at grassroots medical institution of urban and rural residents.
