非布司他与别嘌醇治疗痛风合并高尿酸血症的有效性和安全性比较

Comparison of the effectiveness and safety of febuxostat and allopurinol in the treatment of gout with hyperuricemia

目的分析比较痛风合并高尿酸血症采取非布司他与别嘌醇治疗的有效性及安全性。方法120例痛风合并高尿酸血症患者,按照治疗药物不同分为参照组和研究组,每组60例。参照组患者采用别嘌醇治疗,研究组患者采用非布司他治疗。比较两组患者治疗前后血尿酸、内皮素-1(ET-1)、细胞间粘附因子-1(ICAM-1)水平及治疗效果、不良反应发生情况。结果治疗后,两组患者血尿酸、ET-1、ICAM-1水平均低于本组治疗前,且研究组患者血尿酸(398.03±94.35)μmol/L、ET-1(27.19±8.18)ng/L、ICAM-1(133.44±33.28)ng/L均低于参照组的(445.32±84.52)μmol/L、(33.88±7.18)ng/L、(164.67±41.07)ng/L,差异具有统计学意义(P<0.05)。研究组患者治疗总有效率为91.67%,高于参照组的78.33%,差异具有统计学意义(P<0.05)。研究组患者不良反应发生率为5.00%,低于参照组的13.33%,但差异无统计学意义(P>0.05)。结论临床采取非布司他治疗痛风伴高尿酸血症的效果优于别嘌醇,能够降低患者血尿酸水平,提升治疗有效性和安全性,值得在临床中推广使用。

Objective To analyze and compare the effectiveness and safety of febuxostat and allopurinol in the treatment of gout with hyperuricemia.Methods A total of 120 patients with gout and hyperuricemia were divided into reference group and research group according to different treatment drugs,with 60 cases in each group.Patients in the reference group were treated with allopurinol,and patients in the research group were treated with febuxostat.The levels of blood uric acid,endothelin-1(ET-1),and intercellular adhesion molecule-1(ICAM-1)before and after treatment,therapeutic effect,and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the levels of blood uric acid,ET-1 and ICAM-1 in the two groups were lower than those before treatment in this group,and the serum uric acid(398.03±94.35)μmol/L,ET-1(27.19±8.18)ng/L and ICAM-1(133.44±33.28)ng/L in the research group were lower than(445.32±84.52)μmol/L,(33.88±7.18)ng/L,and(164.67±41.07)ng/L in the reference group.All the differences were statistically significant(P<0.05).The total effective rate in the research group was 91.67%,which was higher than 78.33%in the reference group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the research group was 5.00%,which was lower than 13.33%in the reference group,but the difference was not statistically significant(P>0.05).Conclusion The clinical effect of febuxostat in the treatment of gout with hyperuricemia is better than that of allopurinol,which can reduce the blood uric acid level of patients and improve the efficacy and safety of the treatment.It is worthy of clinical promotion and application.
