补脾益肾汤治疗早期糖尿病肾病的临床分析

Clinical analysis of Bupi Yishen decoction in the treatment of early diabetic nephropathy

目的分析补脾益肾汤治疗早期糖尿病肾病(DN)的临床效果。方法80例早期糖尿病肾病患者,随机分为对照组和观察组,各40例。对照组采用常规治疗,观察组在对照组基础上采用补脾益肾汤治疗。对比两组患者临床疗效、生活质量评分、不良反应发生情况及治疗前后肾功能指标[血清白蛋白、肌酐(Scr)、尿β_(2)微球蛋白(β_(2)-MG)、尿白蛋白排泄率(UAER)]。结果观察组治疗总有效率95.00%明显高于对照组的77.50%,差异具有统计学意义(P<0.05)。治疗后,观察组血清白蛋白(30.17±10.57)g/L高于对照组的(21.75±7.16)g/L,Scr(73.15±15.36)μmol/L、UAER(1.02±0.46)g/24 h、尿β_(2)-MG(4.32±1.06)mg/L均低于对照组的(86.71±15.41)μmol/L、(1.91±0.52)g/24 h、(5.41±1.08)mg/L,差异具有统计学意义(P<0.05)。观察组精神健康、社会功能、生理职能及生理机能评分分别为(86.15±16.79)、(85.26±17.05)、(81.43±18.55)、(82.16±18.17)分,明显高于对照组的(71.25±15.18)、(65.29±15.42)、(65.96±15.73)、(71.23±17.64)分,差异具有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论在早期糖尿病肾病治疗中通过补脾益肾汤的应用能促进患者症状及生活质量的改善,有助于肾功能的恢复,疗效显著且安全,值得临床推广。

Objective To analyze the clinical effect of Bupi Yishen decoction in the treatment of early diabetic nephropathy(DN).Methods A total of 80 patients with early diabetic nephropathy were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with conventional treatment,while the observation group was treated with Bupi Yishen decoction on the basis of the control group.Both groups were compared in terms of clinical efficacy,quality of life score,occurrence of adverse reactions,and renal function indexes serum albumin,serum creatinine(Scr),urinaryβ_(2)microglobulin(β_(2)-MG),and urinary albumin excretion rate(UAER)before and after treatment.Results The total effective rate of the observation group was 95.00%,which was significantly higher than 77.50%of the control group,and the difference was statistically significant(P<0.05).After treatment,the serum albumin(30.17±10.57)g/L in observation group was higher than(21.75±7.16)g/L in control group,and the Scr(73.15±15.36)μmol/L,UAER(1.02±0.46)g/24 h,urinaryβ_(2)-MG(4.32±1.06)mg/L were lower than(86.71±15.41)μmol/L,(1.91±0.52)g/24 h,(5.41±1.08)mg/L in the control group.All the differences were statistically significant(P<0.05).The scores of mental health,social function,role-physical and physiological function in the observation group were(86.15±16.79),(85.26±17.05),(81.43±18.55)and(82.16±18.17)points,which were significantly higher than(71.25±15.18),(65.29±15.42),(65.96±15.73)and(71.23±17.64)points in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Bupi Yishen decoction is effective and safe in the treatment of early diabetic nephropathy,and can promote the improvement of patients'symptoms and quality of life,and contribute to the recovery of renal function,which is worthy of clinical promotion.
