豨蛭络达胶囊联合长春西汀治疗急性期风痰瘀血痹阻脉络型脑梗死的疗效观察及其对血液流变学指标的影响

Effect of Xizhi Luoda capsules combined with vinpocetine on patients of acute cerebral infarction with wind-phlegm and blood stasis in collaterals and its influence on hemorheological indexes

目的:研究豨蛭络达胶囊联合长春西汀治疗急性期风痰瘀血痹阻脉络型脑梗死的临床疗效及其对血液流变学指标的影响。方法:采用前瞻性研究方法,选取2019年2月–2021年2月我院收治的92例急性期风痰瘀血痹阻脉络型脑梗死患者,按照随机数字表法均分为对照组(46例,给予长春西汀治疗)和治疗组(46例,在对照组基础上加用豨蛭络达胶囊),两组均治疗14 d。比较两组中医证候积分、神经功能、日常生活活动能力、血液流变学、临床疗效及不良反应发生情况。结果:治疗后两组中医证候积分、神经功能评分、血液流变学指标均低于治疗前(P<0.05),且治疗组上述指标低于对照组(P<0.05)。治疗后,两组日常生活能力量表-巴士指数(BI)评分均高于治疗前(P<0.05),且治疗组高于对照组(P<0.05)。治疗组临床总有效率为93.48%,高于对照组的76.09%(P<0.05)。两组不良反应发生率无统计学差异(P>0.05)。结论:豨蛭络达胶囊联合长春西汀治疗急性期风痰瘀血痹阻脉络型脑梗死更有利于减轻临床症状,提高患者神经功能及日常生活能力,加速血液畅通,临床效果显著,且安全性高。

Objective:To explore the effect of Xizhi Luoda capsules combined with vinpocetine on patients of acute cerebral infarction with wind-phlegm and blood stasis in collaterals and its hemorheological indexes.Methods:A prospective study was conducted in 92 patients of acute cerebral infarction with wind-phlegm stasis and blood stasis in collaterals in our hospital from February 2019 to February 2021.92 patients were divided into the control group and the therapy group according to the random number table.Patients in the control group and the therapy group were treated with vinpocetine and vinpocetine combined with Xizhi Luoda capsules respectively for 14 days.The scores of traditional Chinese medicine(TCM)syndrome,neurological function,activities of daily living,hemorheology,clinical efficacy and adverse drug reaction(ADR)in two groups were compared after treatment.Results:After treatment,TCM syndrome score,national institutes of health stroke scale(NIHSS)score,hemorheological indexes were all significantly decreased(P<0.05),and all the above indexes in the therapy group were significantly lower than those in the control group(P<0.05).The daily living ability scale-bus index(BI)scores in both groups after treatment were significantly higher than before(P<0.05),and that in the therapy group was significantly higher than that in control group(P<0.05).The total clinical effective rate in the therapy group was 93.48%which was significantly higher than that in the control group(76.09%),P<0.05.There was no significant difference in the incidence of ADR between the two groups(P>0.05).Conclusion:Xizhi Luoda capsules combined with vinpocetine could alleviate clinical symptoms,improve neurological function and ability of daily life,and accelerate blood flow with high safety in patients of acute cerebral infarction with wind-phlegm and blood stasis in collaterals.
