参附注射液联合左西孟旦对急性心力衰竭患者心功能及血管活性物质水平的影响

Effects of Shenfu Injection Combined with Levosimendan on Cardiac Function and Vasoactive Substance Levels in Patients with Acute Heart Failure

目的探讨参附注射液联合左西孟旦治疗急性心力衰竭(AHF)的效果及对患者心功能及血管活性物质水平的影响。方法以2020年2月至2021年11月南阳南石医院收治的102例AHF患者为研究对象,随机分为对照组(51例)与观察组(51例)。对照组静脉滴注左西孟旦,观察组静脉滴注参附注射液联合左西孟旦,均治疗14 d。评价两组疗效,检测两组治疗前后左心室射血分数(LVEF)、心输出量(CO)、每搏输出量(SV)以评估患者心功能,同时比较两组治疗前后6分钟步行试验、血管活性物质[血管紧张素Ⅱ(AngⅡ)、一氧化氮(NO)、内皮素-1(ET-1)],并比较两组不良反应总发生率。结果观察组总有效率较对照组高(P<0.05);治疗后,两组LVEF、CO、SV水平升高,观察组高于对照组(P<0.05);两组6分钟步行试验增长,观察组长于对照组(P<0.05);治疗后,两组血清AngⅡ、ET-1水平较治疗前均降低,NO水平较治疗前升高,且观察组变化程度优于对照组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论参附注射液联合左西孟旦可提高AHF疗效,改善患者心功能,调节血管活性物质。

Objective To investigate the efficacy of Shenfu injection combined with levosimendan in the treatment of acute heart failure(AHF)and its effects on cardiac function and vasoactive substance levels.Methods A total of 102 AHF patients admitted to Nanyang Nanshi Hospital from February 2020 to November 2021 were selected as the research objects,and they were randomly divided into the control group(51 cases)and the observation group(51 cases).The control group was given intravenous infusion of levosimendan,and the observation group was given intravenous infusion of Shenfu injection and levosimendan.Both groups were treated for 14 days.The curative effects of the two groups were evaluated,and the left ventricular ejection fraction(LVEF),cardiac output(CO)and stroke volume(SV)were detected before and after treatment in the two groups to evaluate the cardiac function of the patients.And 6-minute walking distance and vasoactive substance[angiotensinⅡ(AngⅡ),nitric oxide(NO),endothelin-1(ET-1)]levels before and after treatment was compared between the two groups.And the overall incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of LVEF,CO and SV in the two groups were higher than those in the control group(P<0.05).The 6-min walking distance of the two groups increased and the observation group was longer than that of the control group(P<0.05).After treatment,the levels of serum AngⅡand ET-1 in the two groups were lower than those before treatment,and the level of NO was higher than that before treatment,and the degree of change in the observation group was better than that in the control group(P<0.05).There was no statistical difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Shenfu injection combined with levosimendan can improve the efficacy of AHF,improve patients’cardiac function,and regulate the levels of vasoactive substance.
