胃镜治疗消化性溃疡合并急性上消化道出血的疗效分析

Effect of Gastroscopy on Peptic Ulcer Complicated with Acute Upper Gastrointestinal Bleeding

目的:探讨胃镜治疗消化性溃疡合并急性上消化道出血的疗效。方法:采用随机数字表方法作分组设计,将2018年1月~2019年12月本院50例消化性溃疡合并急性上消化道出血患者设为对照组,将同时间段50例消化性溃疡合并急性上消化道出血患者设为实验组;对照组采用三联疗法治疗,实验组加用胃镜治疗方法。比较两组的临床治疗效果、临床指标、血红蛋白含量指标及空腹胃液pH值。结果:实验组患者临床治疗总有效率为96.00%,明显高于对照组的82.00%,P<0.05。实验组患者的止血时间和住院时间分别为(4.50±1.20)d和(8.75±3.25)d,均明显短于对照组患者,P<0.05。实验组和对照组患者治疗后的血红蛋白含量分别为(108.50±19.65)g/L和(96.40±14.75)g/L,均明显高于治疗前,且实验组患者的血红蛋白含量更高,P<0.05。实验组患者治疗24h、治疗48h、治疗72h的空腹胃液pH值高于对照组,P<0.05。结论:对消化性溃疡合并急性上消化道出血患者增加胃镜治疗方法能够缩短止血时间、住院时间,提高血红蛋白含量指标及空腹胃液pH值,促进临床治疗效果的提高。

Objective:To investigate the efficacy of gastroscopy in the treatment of peptic ulcer complicated with acute upper gastrointestinal bleeding.Methods:50 patients with peptic ulcer complicated with acute upper gastrointestinal bleeding in our hospital from January 2018 to December 2019 were set as the control group,and 50 patients with peptic ulcer complicated with acute upper gastrointestinal bleeding in the same period were set as the experimental group;The control group was treated with triple therapy,and the experimental group was treated with gastroscopy.The clinical therapeutic effect,clinical indexes,hemoglobin content and pH value of fasting gastric juice were compared between the two groups.Results:The total effective rate of clinical treatment in the experimental group was 96.00%,which was significantly higher than 82.00%in the control group(P<0.05).The hemostatic time and hospital stay in the experimental group were(4.50±1.20)d and(8.75±3.25)d,respectively,which were significantly shorter than those in the control group(P<0.05).The hemoglobin contents of the experimental group and the control group after treatment were(108.50±19.65)g/L and(96.40±14.75)g/L respectively,which were significantly higher than those before treatment,and the hemoglobin content of the experimental group was higher(P<0.05).The pH value of fasting gastric juice in the experimental group was higher than that in the control group at 24h,48h and 72h after treatment,P<0.05.Conclusion:The addition of gastroscopy in patients with peptic ulcer complicated with acute upper gastrointestinal bleeding can shorten the hemostasis time and hospital stay,improve the index of hemoglobin content and pH value of fasting gastric juice,and promote the improvement of clinical treatment effect.
