β2微球蛋白为缺血性卒中新风险因子的病例对照研究

β2 Microglobulin as a Novel Risk Factor for Ischemic Stroke:a Case-Control Study

摘要 目的:探讨免疫炎症相关血β2微球蛋白(Beta 2 Microglobulin,β2M)与缺血性卒中的关系,及相关指标在缺血性卒中防控中的应用价值.方法:实验样本为安徽省太和县人民医院在2018年5月至2019年5月确诊为急性缺血性脑卒中(Acute Ischemic Stroke,AIS)的住院患者,同一时期在医院进行常规体检的健康人群为对照组.收集患者的一般人口学资料及生化参数,采用倾向评分匹配控制一般人口学和基线资料,采用二元logistic回归分析急性缺血性脑卒中潜在危险因素暴露与发病的风险.结果:通过对207名AIS患者与249名健康对照人群研究发现:两组在年龄、性别、高血压、同型半胱氨酸(Homocysteine,Hcy)、肌酐(Creatinine,Crea)、尿素(Urea)、尿酸(Uric Acid,UA)、总胆固醇(Total Cholesterol,TC)、高密度脂蛋白(High-Density Lipoprotein Cholesterol,HDL)、胱抑素C(Cystatin C,CysC)、C反应蛋白(C Reaction Protein,CRP)和β2M上差异均有统计学意义(P<0.05).匹配后,仅Hcy、Crea、Urea、UA、Cys、CRP和β2M仍存在统计学意义.二元Logistic回归分析发现:Urea(OR=1.220,95%CI:1.005-1.479)、CysC(OR=6.033,95%CI:1.367-26.637)和β2M(OR=2.310,95%CI:1.075-4.963)与AIS均呈显著的正向关联.结论:β2M可能为AIS发病风险新的生物学标志物,相关指标可能成为有应用前景的AIS防治靶点. Objective:To investigate the relationship between immunoinflammation relatedβ2M and ischemic stroke,and the application value of related indicators in the prevention and control of ischemic stroke.Methods:Inpatients diagnosed with AIS in the People’s Hospital of Taihe County,Anhui Province from May 2018 to May 2019 were included retrospectively,and healthy people undergoing routine physical examination in the hospital during the same period were included as the control group.Demographic data and biochemical parameters of the patients were collected.Propensity score matching was used to control the general demographic and baseline data.Binary logistic regression was used to analyze the exposure of potential risk factors and the risk of AIS.Results:A study of 207 AIS patients and 249 healthy controls found that:before matching,there were significant differences in age,sex,hypertension,Hcy,Crea,Urea,UA,TC,HDL,CysC,CRP andβ2M(P<0.05).After matching,only Hcy,Crea,Urea,UA,CysC,CRP andβ2M show a statistical significance between the two group(P<0.05).Binary logistic regression analysis found that Urea(OR=1.220,95%CI:1.005-1.479),CysC(OR=6.033,95%CI:1.367-26.637)andβ2M(OR=2.310,95%CI:1.075-4.963)are significantly positively correlated with AIS.Conclusions:β2M may be a novel biomarker of AIS risk,the related indicators a promising target for AIS prevention and treatment.
