重庆市开州区50岁及以上中老年男性HIV/AIDS患者性需求及性行为调查

Sexual needs and sexual behavior among males infected with HIV/AIDS and aged 50 and above in Kaizhou District,Chongqing

目的了解重庆市开州区50岁及以上男性HIV/AIDS患者性需求及性行为现状,为制定中老年艾滋病防控措施提供科学依据。方法2021年6-12月,抽取重庆市开州区疾病预防控制中心艾滋病VCT门诊和开州区人民医院感染科就诊的50岁及以上男性HIV/AIDS患者200例,进行面对面问卷调查;用SPSS 22.0软件进行统计分析,单因素分析使用χ^(2)检验,多因素分析使用Logistic回归模型,P<0.05认为差异有统计学意义。结果共回收有效问卷199份,有效率99.5%。调查对象平均年龄(64.2±7.9)岁。67.3%(134人)的调查对象自述最近一年有性需求。多因素Logistics回归结果显示,年龄为50~59岁(OR=14.688,95%CI:4.887~44.143)和职业为无业或退休者(OR=3.666,95%CI:1.256~10.706)有性需求的比例更高。70例发生过性行为的调查对象中,90.0%(63人)自述有配偶或固定性伴,最近一次性行为安全套使用比例为98.4%(62/63),其中1例从不使用过安全套。最近1年,发生临时性行为和商业性性行为比例是4.3%(3/70)和8.6%(6/70)。其中商业性性行为性伴数量多为2人及以上。结论重庆市开州区50岁及以上男性HIV/AIDS患者性需求依然普遍存在,且部分患者存在无保护性行为,二次传播风险较高。建议针对中老年HIV/AIDS患者,重点是50~60岁年龄组人群,开展性健康知识教育,树立正确的性观念,持续强化安全套使用意识。

Objective To understand the sexual needs and sexual behaviors among males living with HIV and aged≥50 in Kaizhou District of Chongqing,aiming to provide scientific basis for formulating HIV prevention and control measures to middle-aged and elderly people.Methods From June to December 2021,200 males infected with HIV aged≥50 were selected from the VCT clinic of Kaizhou District Center for Disease Control and Prevention and the department of infectious diseases of People’s Hospital in Kaizhou District for a face-to-face questionnaire survey.SPSS 22.0 software was used for statistical analysis.Chi-square test was used for univariate analysis,and logistic regression model was used for multivariate analysis.P<0.05 was considered statistically significant.Results A total of 199 valid questionnaires were received,with an effective respondent rate of 99.5%.Average age of respondents was 64.2±7.9.67.3%of the respondents reported having sexual needs in the last year.67.3%reported having sexual needs in the current year.Multivariate logistic regression equation analysis showed that respondents who were 50~59 years old(OR=14.688,95%CI:4.887~44.143)and the occupation was unemployed or retired(OR=3.666,95%CI:1.256~10.706)had a higher sexual need.Among the 70 respondents who reported to experience sexual activities,63(90%)reported to have a spouse or a fixed sexual partner,and the rate of using condoms in the most recent sexual activity was 98.4%.One male reported never uses condoms.In the most recent year,the proportion of casual sex and commercial sex was 4.3%and 8.6%,respectively.Among them,the number of commercial sexual partners was 2 and more.Conclusions The sexual needs of male living with HIV and aged≥50 still exist.However,some of them had unprotected sex,which leads to a higher risk of secondary transmission of HIV and other sexual transmitted infections.It is recommended that HIV related intervention should be strengthened among the population,focusing on sexual health knowledge education,promoting healthy sexual activities,and reinforcing the importance of condom use.
