小儿轮状病毒性肠炎并发心肌损害危险因素分析

Analysis of risk factors for myocardial damage associated with rotavirus enteritis in children

目的:探讨轮状病毒性肠炎患儿发生心肌损害的危险因素,为临床诊疗、防治方案制定提供参考。方法:抽取某医院收治的80例轮状病毒性肠炎患儿为研究对象,45例单纯性轮状病毒性肠炎患者作为对照组,35例轮状病毒性肠炎并发心肌损害患者作为实验组,回顾性分析诱发心肌损害的危险因素。结果:单因素分析结果显示,实验组患儿呕吐发生率、重度脱水发生率、代谢性酸中毒发生率、WBC水平、血Na水平、血K水平、RDW水平与对照组差异存在统计学意义(P<0.05);多因素分析结果显示,轮状病毒性肠炎发生心肌损害的独立危险因素有呕吐、重度脱水、代谢性酸中毒、低水平K、高水平RDW。结论:呕吐、脱水程度、代谢性酸中毒及实验室指标(K、RDW)异常均可能导致轮状病毒性肠炎患儿发生心肌损害,故医护人员必须予以高度重视,并做好预防措施,尽可能降低心肌损害发生风险,保障患儿生命健康。

Objective:To analyze the risk factors of myocardial damage in children with rotavirus enteritis,and to provide reference for clinical diagnosis and treatment plan formulation.Methods:Eighty children with rotavirus enteritis treated in hospital were selected as research subjects,45 patients with simple rotavirus enteritis as control group,and 35 patients with rotavirus enteritis complicated by myocardial damage were selected as experimental group.The risk factors inducing myocardial damage were retrospectively analyzed.Results:univariate analysis showed that the incidence of vomiting,severe dehydration,metabolic acidosis,WBC,blood K,and RDW from the control group(P<0.05);multivariate analysis showed that independent risk factors for myocardial injury in rotaviral enteritis were vomiting,severe dehydration,metabolic acidosis,low level K,and high RDW.Conclusion:The degree of vomiting,dehydration,metabolic acidosis and abnormal laboratory indicators(K,RDW)may lead to myocardial damage in children with rotavirus enteritis,so medical staff must pay great attention to it and take preventive measures to reduce the risk of myocardial damage as far as possible and ensure the life and health of children.
