摘要
目的探讨基于盆腔非增强CT(NECT)的主髂动脉系统钙化评分在高风险待肾移植患者术前的应用效果。方法收集2020年1月至2021年12月首都医科大学宣武医院收治的102例高风险待肾移植患者的临床资料,分析患者的影像学资料,观察髂动脉斑块的分布情况,包括斑块总数量、斑块性质。比较两种检查方法下的斑块情况,包括髂动脉管腔面积、斑块面积、斑块负荷。比较不同狭窄程度髂动脉的钙化评分。分析髂动脉狭窄程度与髂动脉钙化斑块评分的相关性。结果髂动脉共204支,其中,彩色多普勒超声(CDS)检查出钙化斑块302处,非钙化斑块327处;NETC检查出钙化斑块311处,非钙化斑块328处。两种检查方法对髂动脉斑块的检查结果比较,差异无统计学意义(P>0.05)。两种检查方法下的斑块面积、斑块负荷比较,差异均无统计学意义(P>0.05)。结论基于NECT的主髂动脉系统钙化评分与CDS检查结果具有较好的一致性,对高风险待肾移植患者术前的心血管风险评估具有重要意义。
Objective To explore the application of aortoiliac arterial system calcification score based on nonenhanced pelvic computer tomography(NECT)in high-risk patients awaiting kidney transplantation before operation.Method The clinical data of 102 high-risk patients awaiting kidney transplantation in Xuanwu Hospital,Capital Medical University from January 2020 to December 2021 were collected.The imaging data was analyzed to observe the distribution of plaques in iliac artery,including total number and nature of plaques,and to compare the plaques detected by two methods,including lumen area of iliac arteries,plaque area and burden of plaques.The calcified plaque scores in iliac artery with different stenosis was compared to analyze the correlation of stenosis in iliac artery with calcified plaque score.Result There were 204 iliac arteries,302 calcified plaques and 327 non-calcified plaques were detected by color Doppler ultrasonography(CDS),and 311 calcified plaques and 328 non-calcified plaques were detected by NECT.There was no significant difference for the results of iliac artery plaques detected by two methods(P>0.05);there was no significant difference for plaque area and plaque burden detected by two methods(P>0.05).Conclusion The NECT-based aortoiliac arterial system calcification score has a good consistency with the results of CDS,which is of great significance for the cardiovascular risk evaluation of high-risk patients awaiting kidney transplantation.
