探讨在直肠癌临床术前分期中应用CT影像诊断的效果

Exploring on the Effect of CT Imaging Diagnosis in Clinical Preoperative Staging of Rectal Cancer

目的探讨在直肠癌临床术前分期中应用电子计算机断层扫描(computed tomography,CT)影像诊断的效果。方法选取2020年1月—2021年8月滕州市中心人民医院收治的直肠癌患者120例,均接受多层螺旋CT检查,将病理检查结果作为“金标准”,分析CT影像评估直肠癌临床术前分期的效果,计算CT影像与病理检查结果的一致性。结果CT影像诊断直肠癌分期的总准确率为88.33%;CT影像诊断直肠癌T_(1)、T_(2)、T_(3)、T_(4)的敏感度分别为78.57%、80.65%、94.83%、88.24%,特异度分别为100.00%、95.51%、90.32%、96.19%,阳性预测值分别为100.00%、86.21%、87.30%、88.24%,阴性预测值分别为97.25%、93.41%、94.74%、98.06%;CT影像与病理检查结果的一致性Kappa值为0.820。结论直肠癌临床术前分期中应用CT影像诊断的效果确切,可为后续治疗的开展提供参考。

Objective To investigate the effect of computed tomography(CT)imaging in the clinical preoperative staging of rectal cancer.Methods A total of 120 patients with rectal cancer admitted to Tengzhou Central People′s Hospital from January 2020 to August 2021 were selected,and all patients received multi-slice spiral CT examination.The results of pathological examination were used as the"gold standard"to analyze the effect of CT images in evaluating the clinical preoperative staging of rectal cancer.The consistency of CT images and pathological examination results was calculated.Results The overall accuracy rate of CT imaging in diagnosing rectal cancer staging was 88.33%;the sensitivity of CT imaging for diagnosing rectal cancer TT_(1),T_(2),T_(3)and T_(4)were 78.57%,80.65%,94.83%,and 88.24%,respectively,and the specificity was 100.00%,95.51%,90.32%,96.19%,the positive predictive values were 100.00%,86.21%,87.30%,88.24%,and the negative predictive values were 97.25%,93.41%,94.74%,and 98.06%,respectively.The Kappa value of the consistency between CT imaging and pathological examination results was 0.820.Conclusion The effect of CT imaging diagnosis in clinical preoperative staging of rectal cancer is definite,which can provide reference for the development of follow-up treatment.
