低剂量薄层CT在肺部结节患者良恶性中的价值分析

摘要 目的评价在肺部结节患者良恶性诊断中应用低剂量薄层CT的价值。方法选择临朐县中医院在2020年6月—2021年6月期间接诊的40例肺部结节患者,分别采用常规剂量薄层CT诊断、低剂量薄层CT诊断,分析低剂量薄层CT在肺部结节患者良恶性中的价值。结果低剂量薄层CT的敏感度为81.25%,特异度为79.17%,准确率为80.00%。常规剂量薄层CT的敏感度为75.00%,特异度为70.83%,准确率为72.50%。低剂量薄层CT与常规剂量薄层CT的特异度、灵敏度与准确率相比差异无统计学意义(P>0.05)。低剂量薄层CT的辐射剂量低于常规剂量薄层CT诊断,CTDIw(26.65±4.23)mGy、DLP(1.23±0.12)m Gy/cm、MERD(0.72±0.15)mGy/cm均小于常规剂量薄层CT诊断(180.25±12.69)mGy、(9.13±0.52)mGy/cm、(1.32±0.22)m Gy/cm,差异有统计学意义(t=61.003、166.547、54.210,P<0.001)。结论在肺部结节患者良恶性诊断中应用低剂量薄层的效果显著,与常规剂量检查和病理活检无明显差异,但是可以降低对患者的辐射损伤,安全性较高,值得在临床中予以推广使用。 Objective To evaluate the value of low-dose thin-slice CT in the diagnosis of benign and malignant pulmonary nodules.Methods Forty patients with pulmonary nodules admitted to Linqu County Hospital of Traditional Chinese Medicine from June 2020 to June 2021 were selected.Conventional-dose thin-slice CT and low-dose thin-slice CT were used for diagnosis,respectively,and the value of low-dose thinslice CT in benign and malignant pulmonary nodules was analyzed.Results The sensitivity of low-dose thin-slice CT was 81.25%,the specificity was 79.17%,and the accuracy was 80.00%.The sensitivity of conventional dose thin-slice CT was 75.00%,the specificity was 70.83%,and the accuracy was 72.50%,there was no statistically significant difference in the specificity,sensitivity and accuracy of conventional dose thin-slice CT(P>0.05).The radiation dose of low-dose thin-slice CT was lower than that of conventional-dose thin-slice CT,CTDIw(26.65±4.23)mGy,DLP(1.23±0.12)m Gy/cm,and MERD(0.72±0.15)m Gy/cm were all lower than those of conventional dose thin-slice CT diagnosis(180.25±12.69)mGy,(9.13±0.52)m Gy/cm,(1.32±0.22)mGy/cm,the difference was statistically significant(t=61.003,166.547,54.210,P<0.001).Conclusion The application of low-dose thin layer in the diagnosis of benign and malignant pulmonary nodules has a significant effect,which is not significantly different from routine dose examination and pathological biopsy.However,it can reduce radiation damage to patients and has high safety,so it is worthy of promotion and use in clinical practice.
