健脾益气法联合甘露聚糖肽片对老年结直肠癌患者术后免疫应激水平及疗效的影响

Effects of Jianpi Yiqi Method Combined with Mannan Peptide Tablets on Postoperative Immune Stress Level and Curative Effect in Elderly Patients with Colorectal Cancer

目的探究并分析健脾益气法联合甘露聚糖肽片对老年结直肠癌患者术后免疫应激水平及疗效的影响。方法选取2016年8月—2019年8月泰安市中医二院收治的老年结直肠癌者100例,随机分为两组,各50例。对照组患者给予甘露聚糖肽片治疗,观察组患者在对照组患者基础上给予健脾益气法治疗。观察并记录两组患者术前一般情况,治疗前后mHLA-DR、sEcad、TNF-a、IL-6水平,临床疗效及不良反应。结果治疗后1个月,观察组患者mHLA-DR水平显著高于对照组[(87.13±16.43)%vs(74.32±13.31)%],差异有统计学意义(t=4.28,P<0.05);治疗1个月观察组患者sE-cad水平显著低于对照组[(1931±165)μg/L vs(2152±157)μg/L],差异有统计学意义(t=6.86,P<0.05);治疗后1周、治疗2周、治疗1个月观察组患者TNF-a含量明显低于对照组,差异有统计学意义(P<0.05);治疗后1周、治疗2周、治疗1个月观察组患者IL-6含量明显低于对照组,差异有统计学意义(P<0.05);观察组患者术后肠道功能恢复时间显著短于对照组,差异有统计学意义(P<0.05),两组患者未见严重不良反应,两组患者不良反应发生率比较,差异无统计学意义(χ^(2)=0.06,P>0.05)。结论健脾益气法联合甘露聚糖肽片在改善老年结直肠癌患者术后免疫应激功能方面有一定临床价值。

Objective To explore and analyze the effect of Jianpi Yiqi method combined with mannan peptide tablets on postoperative immune stress level and curative effect in elderly patients with colorectal cancer.Methods A total of 100 elderly patients with colorectal cancer who were treated in Tai'an City Second Hospital of Traditional Chinese Medicine from August 2016 to August 2019 were selected and randomly divided into two groups,50 cases in each group.The patients in the control group were treated with mannan peptide tablets,and the patients in the observation group were treated with the method of strengthening the spleen and nourishing qi on the basis of the patients in the control group.The preoperative general conditions,mHLA-DR,sE-cad,TNF-a,IL-6 levels,clinical efficacy and adverse reactions of the two groups of patients before and after treatment were observed and recorded.Results One month after treatment,the mHLA-DR level in the ob⁃servation group was significantly higher than that in the control group[(87.13±16.43)%vs(74.32±13.31)%],the difference was statistically significant(t=4.28,P<0.05).After 1 month of treatment,the level of sE-cad in the observation group was significantly lower than that in the control group[(1931±165)μg/L vs(2152±157)μg/L],the difference was statistically significant(t=6.86,P<0.05).The levels of TNF-a in the observation group were significantly lower than those in the control group at 1 week,2 weeks and 1 month after treatment,the difference was statistically significant(P<0.05).The levels of IL-6 in the observation group were significantly lower than those in the control group at 1 week,2 weeks and 1 month after treatment,the difference was statistically significant(P<0.05).The postoperative bowel function recovery time of the observation group was significantly shorter than that of the control group,the difference was statistically significant(P<0.05),no se⁃rious adverse reactions were found in the two groups,and there was no statistically significant difference in the incidence of adverse reactions between the two groups(χ^(2)=0.06,P>0.05).Conclusion Jianpi Yiqi method combined with mannan peptide tablets has certain clinical value in improving postoperative immune stress function in elderly patients with colorectal cancer.
