幽门螺杆菌阳性胃溃疡糖脂代谢、CRP水平变化及其与含铋剂四联疗法疗效的关系分析

Changes of Glucose and Lipid Metabolism and CRP Levels in Helicobacter Pyloriposi-tive Gastric Ulcer and Their Relationship with the Curative Effect of Bismuthcontaini-ng Quadruple Therapy

目的探究幽门螺杆菌阳性胃溃疡糖脂代谢、C反应蛋白(CRP)水平变化及其与含铋剂四联疗法疗效的关系。方法选取2020年7月—2021年6月深圳市罗湖区人民医院收治的幽门螺杆菌阳性胃溃疡患者200例为研究对象,均采用含铋剂四联疗法治疗。比较治疗前后血浆CRP、总胆固醇(TG)、三酰甘油(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)水平及空腹血糖(FPG)、餐后2 h血糖(2 hPG)和糖化血红蛋白(HbAlc)水平。结果200例患者连续治疗2周后完全痊愈的72例,治疗有效113例,治疗无效15例,总有效率为92.5%;治疗2周后,治疗有效的胃溃疡患者的血浆CRP(6.82±1.44)mg/L、TG(3.46±0.75)mmol/L、LDL(3.54±0.65)mmol/L、FPG(5.83±0.89)mmol/L、2 hPG(9.72±1.06)mmol/L、HbAlc(6.01±0.84)%均低于治疗无效的胃溃疡患者,差异有统计学意义(t=2.894、3.817、3.932、2.370、3.789、4.157,P<0.05),且不同治疗效果患者的血浆TC和HDL水平间比较差异无统计学意义(P>0.05)。结论含铋的四联疗法在幽门螺杆菌阳性的胃溃疡患者中显示出较好的治疗效果,含铋的四联疗法治疗有效可能与其能够降低CRP、TG、LDL、FPG、2 hPG和HbAlc水平,改善炎症状态和糖脂代谢有关。

Objective To investigate the changes of glucose and lipid metabolism and C-reactive protein(CRP)levels in Helicobacter pyloripositive gastric ulcer and their relationship with the curative effect of bismuth-containing quadruple therapy.Methods A total of 200 patients with Helicobacter pylori-positive gastric ulcer who were admitted from July 2020 to June 2021 were selected as the research objects,and all of them were treated with bismuth-containing quadruple therapy.The levels of plasma CRP,total cholesterol(TG),triacylglycerol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL)levels and fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG)and glycosylated hemoglobin(HbAlc)levels were compared before and after treatment.Results Of the 200 patients,72 were completely cured af⁃ter 2 weeks of continuous treatment,113 were effective and 15 were ineffective,with a total effective rate of 92.5%.After 2 weeks of treat⁃ment,the plasma CRP(6.82±1.44)mg/L,TG(3.46±0.75)mmol/L,LDL(3.54±0.65)mmol/L,FPG(5.83±0.89)mmol/L,2 hPG(9.72±1.06)mmol/L,and HbAlc(6.01±0.84)%of the effective gastric ulcer patients,were lower than those of gastric ulcer patients with ineffective treatment,and the difference was statistically significant(t=2.894,3.817,3.932,2.370,3.789,4.157,P<0.05).There was no statistically sig⁃nificant difference in plasma TC and HDL levels between patients with different treatment effects(P>0.05).Conclusion Bismuth-containing quadruple therapy shows a good therapeutic effect in patients with Helicobacter pylori-positive gastric ulcer.The effectiveness of bismuth-containing quadruple therapy may be related to its ability to reduce CRP, TG, LDL, FPG, 2 hPG and HbAlc levels, improve inflammatorystate and glucose and lipid metabolism.
