探讨骨膨胀裂开法结合引导骨组织再生技术在种植牙手术中的应用

Exploring the Application of Bone Expansion and Dehiscence Combined with Guided Bone Tissue Regeneration in Dental Implant Surgery

目的探讨种植牙手术患者采用骨膨胀裂开法结合引导骨组织再生技术治疗对临床效果的影响。方法选取2019年3月—2021年5月在济南市第三人民医院行种植牙手术的患者82例,采用随机摸球法的方法分成A组(n=41)与B组(n=41)。B组采用引导骨组织再生技术治疗,A组采用骨膨胀裂开法结合引导骨组织再生技术治疗。对两组临床疗效、牙周指标、修复效果、生活质量进行比较。结果A组治疗总有效率(97.56%)较B组(80.49%)明显更高,差异有统计学意义(χ^(2)=4.493,P<0.05);治疗后,两组牙槽嵴顶唇舌向水平宽度明显提高,差异有统计学意义(P<0.05),A组较B组明显更高,差异有统计学意义(P<0.05);治疗后两组牙周附着水平(PAL)明显降低,差异有统计学意义(P<0.05),A组较B组明显更低,差异有统计学意义(P<0.05);A组修复效果(固位效果、语言功能、咀嚼功能、美观度、耐受度、舒适度)评分较B组明显更高,差异有统计学意义(P<0.05);治疗后两组生活质量明显提高,差异有统计学意义(P<0.05),A组评分较B组明显更高,差异有统计学意义(P<0.05)。结论骨膨胀裂开法结合引导骨组织再生技术应用于种植牙手术治疗中能够提高临床疗效、口腔修复效果与患者生活质量,改善患者牙周指标。

Objective To investigate the effect of bone expansion and dehiscence combined with guided bone tissue regeneration on the clinical effect of patients undergoing dental implant surgery.Methods 82 patients who underwent dental implant surgery in Jinan Third People's Hos⁃pital from March 2019 to May 2021 were selected and divided into group A(n=41)and group B(n=41)by random touch-ball method.Group B was treated with guided bone tissue regeneration technology,and group A was treated with bone expansion and dehiscence combined with guided bone tissue regeneration technology.The clinical efficacy,periodontal index,restoration effect and quality of life were compared be⁃tween the two groups.Results The total effective rate of group A(97.56%)was significantly higher than that of group B(80.49%),the differ⁃ence was statistically significant(χ^(2)=4.493,P<0.05).After treatment,the width of the alveolar crest in the labial and lingual direction of the two groups was significantly increased,the difference was statistically significant(P<0.05),and those in group A was significantly higher than in group B,the difference was statistically significant(P<0.05).After treatment,the periodontal attachment level(PAL)in the two groups was significantly decreased,the difference was statistically significant(P<0.05),and those in group A was significantly lower than in group B,the difference was statistically significant(P<0.05).The repair effect(retention effect,language function,masticatory function,aesthetics,tolerance,comfort)score of group A was significantly higher than that of group B,the difference was statistically significant(P<0.05).After treat⁃ment,the quality of life in the two groups was significantly improved,the difference was statistically significant(P<0.05),and the score of group A was significantly higher than that of group B,the difference was statistically significant(P<0.05).Conclusion Bone expansion and de⁃hiscence combined with guided bone tissue regeneration in the treatment of dental implants can improve the clinical efficacy,prosthodontic effect and quality of life of patients,and improve the periodontal indexes of patients.
