原发性肾病综合征血瘀证与脂质代谢紊乱的关系分析

Analysis of Relationship Between Blood Stasis Syndrome of Primary Nephrotic Syndrome and Lipid Metabolism Disorder

目的:分析原发性肾病综合征血瘀证与脂质代谢紊乱的关系。方法:收集120例原发性肾病综合征患者的临床资料,根据是否合并血瘀证将患者分为观察组80例和对照组40例。比较2组血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)]水平;分析观察组血瘀证积分与血脂指标的相关性;随访6个月,统计患者6个月后临床情况,分析影响疗效的相关因素。结果:治疗前,观察组TG、TC、LDL均高于对照组,HDL低于对照组,差异均有统计学意义(P<0.05)。观察组血瘀证积分与TG、TC、LDL水平均呈正相关性(P<0.05),与HDL水平无明显相关性(P>0.05)。Logistic分析发现TC(OR=2.33,P<0.05)、TG(OR=5.47,P<0.05)与LDL(OR=4.66,P<0.05)是影响患者疗效的危险因素。结论:原发性肾病综合征血瘀证积分与TC、TG、LDL等血脂指标密切相关,且脂质代谢紊乱是影响患者疗效的危险因素。

Objective: To analyze the relationship between the score of blood stasis syndrome of primary nephrotic syndrome and lipid metabolism disorder. Methods:The clinical data of 120 patients with primary nephrotic syndrome were collected and divided into the observation group and the control group according to whether there was blood stasis syndrome,with 80 and 40 cases in each group respectively.The levels of blood lipid indexes,including total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL) and high density lipoprotein(HDL),were compared between the two groups. The correlation between the score of blood stasis syndrome and blood lipid indexes in the observation group was analyzed. The patients were followed up for six months,and the clinical situation of the patients after six months was analyzed. Results:Before treatment,the levels of TG,TC and LDL in the observation group were higher than those in the control group,and the HDL level was lower,differences being significant(P<0.05). The score of blood stasis syndrome was positively correlated with the levels of TG, TC and LDL in the observation group(P<0.05), but had no significant correlation with the HDL level(P>0.05). According to Logistic analysis,TC(OR=2.33,P<0.05),TG(OR=5.47,P<0.05) and LDL(OR=4.66,P<0.05) were risk factors affecting the therapeutic effect on patients. Conclusion: The score of blood stasis syndrome of primary nephrotic syndrome is closely related to serum lipid indexes such as TC,TG and LDL,and lipid metabolism disorder is a risk factor affecting the therapeutic effect on patients.
