扶正祛瘀汤联合化疗治疗中晚期胃癌临床研究

Clinical Study on Fuzheng Quyu Tang Combined with Chemotherapy for Advanced Gastric Cancer

目的:观察扶正祛瘀汤联合化疗治疗中晚期胃癌的临床疗效及对免疫功能及生活质量的影响。方法:将90例中晚期胃癌患者按随机数字表法分为对照组及观察组各45例,对照组予FOLFOX化疗,观察组在对照组基础上加服扶正祛瘀汤。比较2组临床疗效,并比较2组治疗前后生活质量及免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫细胞亚群CD3^(+)、CD4^(+)、CD8^(+)、NK水平,计算CD4^(+)/CD8^(+)水平。结果:观察组总有效率62.22%,高于对照组35.56%(P<0.05)。与同组治疗前比较,治疗后2组IgA、IgG、IgM水平均降低(P<0.05);与对照组治疗后比较,观察组治疗后IgA、IgG、IgM水平均较低(P<0.05)。与同组治疗前比较,对照组治疗后NK降低(P<0.05),观察组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK及生活质量各指标评分均升高(P<0.05),CD8^(+)降低(P<0.05);与对照组治疗后比较,观察组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK及生活质量各指标评分均较高(P<0.05),CD8^(+)较低(P<0.05)。结论:扶正祛瘀汤联合化疗对中晚期胃癌患者具有较好的临床疗效,可改善生存质量,其作用机制可能和改善免疫功能,增强抗病能力有关。

Objective:To observe the clinical effect of the therapy of Fuzheng Quyu tang combined with chemotherapy on advanced gastric cancer and its effect on immune function and quality of life.Methods:A total of 90 patients with advanced gastric cancer were divided into the control group and the observation group according to the random number table method,with 45 cases in each group.The control group was treated with FOLFOX chemotherapy,and the observation group was additionally treated with Fuzheng Quyu tang orally based on the treatment of the control group.The clinical effects were compared between the two groups.Before and after treatment,the quality of life and the levels of immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)and immune cell subsets,including CD3^(+),CD4^(+),CD8^(+)and NK were compared between the two groups,and the level of CD4^(+)/CD8^(+)was calculated.Results:The total effective rate was 62.22%in the observation group,higher than that of 35.56%in the control group(P<0.05).After treatment,the levels of IgA,IgG and IgM in the two groups were decreased when compared with those in the same group before treatment(P<0.05);after treatment,the above three levels in the observation group were lower than those in the control group(P<0.05).Compared with the same group before treatment,the level of NK in the control group was decreased(P<0.05),the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK and score of each index of quality of life in the observation group were increased(P<0.05),and the level of CD8^(+)in the observation group was decreased(P<0.05);after treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK and score of each index of quality of life in the observation group were higher than those in the control group(P<0.05),and the level of CD8^(+)in the observation group was lower(P<0.05).Conclusion:The therapy of Fuzheng Quyu tang combined with chemotherapy has a good clinical effect on patients with advanced gastric cancer,which can improve the quality of life,and its mechanism may be related to improving the immune function and enhancing the disease resistance.
