Exploration on the Optimization of Night First Aid Process for Hospital Emergency Patients

目的 探索医院急诊患者晚夜间急救流程的优化,提升医院安全风险管理能力。方法 2021年1—10月成立医疗失效模式与效应分析(healthcare failure mode and effect analysis,HFMEA)小组,对急诊患者晚夜间急救过程进行追踪并绘制流程图,应用头脑风暴法找出每个步骤可能的失效模式并计算风险指数,随后实施决策树分析找出需要重点改善的失效模式,应用根因分析法发现系统原因并制定行动策略,包括加强制度管理、全面开展培训以及建立院内快速反应小组。结果 采取改善行动后各项风险指数均较改善前减少,改善率50.0%~75.0%,效果明显。结论 HFMEA可提升急诊晚夜间急救质量和安全性,保障患者安全。

Objective To explore the optimization of night first aid process for hospital emergency patients and improve the ability of hospital safety risk management. Methods From January to October 2021 healthcare failure mode and effect analysis(HFMEA) team was established to track the emergency process of emergency patients at night and draw the flow chart. The brainstorming method was used to find out the possible failure modes of each step and calculate the risk index, and then decision tree analysis was implemented to find out the failure modes that need to be improved. Root cause analysis was utilized to find out the system causes and formulate action strategies, including strengthening system management, carrying out comprehensive training and establishing a rapid response team in the hospital. Results After taking improvement action, all risk indexes were reduced compared with those before improvement, the improvement rate was 50.0% to 75.0%, and the effect was obvious. Conclusion HFMEA can improve the quality and safety of emergency night first aid and ensure the safety of patients.
