ERAS在直肠癌腹腔镜手术中的应用效果及外科Apgar评分、CR-POSSUM评估分析

ERAS Application Effect,Surgical Apgar Score and CR-POSSUM Evaluation Analysis in Patients with Rectal Cancer Laparoscopic Surgery

目的探究ERAS在直肠癌腹腔镜手术中的应用效果及外科Apgar评分及不良反应。方法选取直肠癌患者87例,采用随机数字法分为观察组(45例)与对照组(42例)。观察组采用加速康复外科(ERAS)联合腹腔镜直肠癌手术,对照组采取传统治疗方法联合腹腔镜直肠癌手术,观察2组患者围手术各项指标。结果观察组排便时间、术后下床时间、住院费用、住院天数均低于对照组,差异具有统计学意义(P<0.05)。观察组外科Apgar评分高于对照组,差异具有统计学意义(P<0.05)。随访患者1年,观察组术后死亡率(2.22%)低于对照组术后死亡率(14.28%),差异具有统计学意义(P<0.05)。结论ERAS联合腹腔镜直肠癌根治术减少了患者住院时间,促进了患者机体恢复,增加了治疗效果,提高了治疗的安全系数,有效减少患者并发症发生风险,改善了患者生存质量。

Objective To explore the application effect of eras in laparoscopic surgery for rectal cancer,surgical Apgar score and adverse reactions.Methods 87 patients with rectal cancer were randomly divided into the observation group(45 cases)and the control group(42 cases).The observation group was treated with accelerated rehabilitation surgery(ERAS)combined with laparoscopic rectal cancer surgery,and the control group was treated with traditional treatment combined with laparoscopic rectal cancer surgery.The perioperative indexes of the 2 groups were observed.Results The time of defecation,the time of getting out of bed after operation,the cost of hospitalization and the days of hospitalization in the observation group were significantly lower than those in the control group(P<0.05).The surgical Apgar score of the observation group was significantly higher than that of the control group(P<0.05).After 1-year follow-up,the postoperative mortality of the observation group(2.22%)was lower than that of the control group(14.28%),and the difference was statistically significant(P<0.05).Conclusion Eras combined with laparoscopic radical resection for rectal cancer can reduce the hospitalization time,promote the recovery of patients,increase the treatment effect,improve the safety factor,effectively reduce the risk of complications and improve the quality of life of patients.
