摘要
目的分析前列腺癌(PCa)患者经耻骨后前列腺癌根治术(RRP)治疗后尿控恢复的相关影响因素及临床价值。方法选择行RRP治疗的120例PCa患者作为研究对象,按照术后尿控是否恢复分为恢复组(64例)和未恢复组(56例)。收集患者年龄、体重指数(BMI)、前列腺质量、基础疾病、术前下尿路症状(LUTS)等一般资料,分析术后尿控恢复的相关影响因素。结果2组基础疾病、术前PSA水平、分化程度、手术时间比较,差异无统计学意义(P>0.05);2组年龄、BMI、前列腺质量、术前LUTS、肿瘤分期、术中出血量、保留前列腺尿道、保留膀胱颈、保留血管神经根束比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析,年龄、前列腺质量、术前LUTS、肿瘤分期、保留前列腺尿道、保留膀胱颈、保留血管神经根束均是影响术后尿控恢复的因素。结论年龄、前列腺质量、术前LUTS、肿瘤分期、保留前列腺尿道、保留膀胱颈、保留血管神经根束均是RRP治疗PCa患者术后尿控恢复的相关影响因素,对临床治疗具有较高的价值。
Objective To analyze the influencing factors and clinical value of urinary control after radical prostate cancer(RRP)in patients with prostate cancer(PCa).Methods 120 PCa patients treated with RRP were selected as study subjects and divided into recovery group(64)and unrecovered group(56)according to whether urinary control recovered after surgery.General data on patient age,body mass index(BMI),prostate quality,underlying disease,preoperative lower urinary tract symptoms(LUTS)were collected to analyze the relevant influencing factors of postoperative urinary control.Results Comparison of basic disease,preoperative PSA level,differentiation,surgery,P>0.05),age,BMI,prostate quality,preoperative LUTS,tumor stage,intraoperative bleeding,prostate urethra retention,bladder neck and vascular nerve root tract retention(P<0.05).In multivariate Logistic regression analysis,age,prostate quality,preoperative LUTS,tumor stage,prostate urethra,bladder neck,and vascular nerve root tract were all influencing factors affecting postoperative urine control.Conclusion Age,prostate quality,preoperative LUTS,tumor stage,prostate urethra,bladder neck and vascular nerve root tract are all relevant influencing factors for RRP treatment of PCa patients,which can provide reliable basis for clinical treatment and have high clinical value.
