盆底超声评估不同分娩方式对盆底功能的影响

Effect of different delivery methods on pelvic floor function assessed by pelvic floor ultrasound

目的研究盆底超声评估不同分娩方式对盆底功能的影响。方法选择龙口市人民医院2018年10月至2021年9月收治的169例产妇作为研究对象,其中68例行剖宫产,101例行阴道分娩,另外再选择30例未生育的正常女性,分别为剖宫产组、阴道分娩组以及对照组。对三组产妇分别进行妇科检查、盆底超声及盆底肌力评估,分析三组产妇的盆底超声结果以及盆底肌力变化情况。结果在静息状态下以及Valsalva动作,以阴道分娩组的膀胱颈角度最大,阴道分娩组的膀胱旋转角度以及膀胱颈移动度高于其他两组,差异均有统计学意义(P<0.05)。结论不同的分娩方式对女性的盆底功能造成不同程度影响,以阴道分娩的方式对盆底功能的损伤程度相对较大。建议在生产后,对行阴道分娩以及剖宫产分娩的女性早期进行盆底功能训练,进一步降低对盆底功能的影响,从而改善机体功能。

Objective To study the effect of different delivery methods on pelvic floor function assessed by pelvic floor ultrasound.Methods A total of 169 puerpera admitted to Longkou People’s Hospital from October 2018 to September 2021 were selected as the research subjects,including 68 cases of cesarean section,101 cases of vaginal delivery,and another 30 cases of nulliparous normal women were selected,as cesarean section group,vaginal delivery group and control group,respectively.They were given gynecological examination,pelvic floor ultrasound and pelvic floor muscle strength assessment,and the pelvic floor ultrasound results and pelvic floor muscle strength changes of the three groups were analyzed.Results In the resting state and Valsalva maneuver,the vaginal delivery group had the largest bladder neck angle,and the vaginal delivery group had higher bladder rotation angle and bladder neck mobility than the other two groups,the differences were statistically significant(P<0.05).Conclusion Different delivery methods will affect the pelvic floor function of women to varying degrees.Among them,vaginal delivery has a relatively large degree of damage to pelvic floor function.It is recommended to carry out pelvic floor function training for women undergoing vaginal delivery and cesarean section delivery at an early stage to further reduce the impact on pelvic floor function and improve body function.
