耳鸣音调与响度改变的相关影响因素研究

Related Factors Affecting the Pitch and Loudness of Tinnitus

目的 分析影响耳鸣音调与响度改变的相关因素,为耳鸣机制研究提供临床资料。方法 选取2014年12月~2018年5月南方医科大学南方医院耳鼻咽喉头颈外科耳鸣患者400例。收集患者的一般资料,包括年龄、性别、耳鸣相关特点(耳鸣侧别、时间、音调及音调响度是否改变)、听力损失程度(相应听力损失程度与言语频率听力损失程度)、听力损失类型、耳鸣残疾量表得分(tinnitus handicap inventory,THI)、抑郁(self-rating anniety scale,SAS)及焦虑量表(self-rating de pression scale,SDS)得分,进行统计学分析。结果 性别是耳鸣音调改变的独立影响因素(P=0.030),耳鸣侧别(P=0.009)、耳鸣持续时间(P=0.040)、性别(P=0.025)是耳鸣响度改变的独立影响因素。结论 耳鸣音调改变与性别具有相关性,耳鸣响度改变与耳鸣侧别、持续时间、性别具有相关性。

Objective To observe and analyze related factors that may affect the pitch and loudness of tinnitus, and to provide enough clinical data for the study of mechanism of tinnitus. Methods 400 tinnitus suffers in Department of OtorhinolaryngologyHead and Neck Surgery, Nan Fang Hospital, Southern Medical University from December 2014 to May 2018 were selected as the research objects. To collect the general date of patients with tinnitus,the following parameters were considered: age, gender, the characteristics of tinnitus(tinnitus laterality,tinnitus duration, subjective judgment of pitch and loudness change or not), degree of hearing loss(corresponding tinnitus pitch and speech frequency), type of hearing loss, scores of Tinnitus Handicap Inventory, SelfRating Anxiety Scale, Self-Rating Depression Scale. Results Gender(P=0.030) is an independent factor that influence the pitch of tinnitus.The independent factors related to tinnitus loudness include tinnitus laterality(P=0.009), tinnitus duration(P=0.040) and gender(P=0.025). Conclusion There is a correlation between tinnitus pitch change and gender, and the change in tinnitus loudness has a correlation with tinnitus laterality,tinnitus duration and gender.
