1例听阈正常伴感音神经性听力损失症状患者的临床干预

Clinical Intervention of Hearing Aids in a Patient with Normal Hearing Thresholds with Symptoms of Sensory Hearing Loss

本文对1例听阈正常但伴有感音神经性听力损失症状(噪声下言语分辨力下降、听觉过敏、耳鸣、声源定位困难等)的患者使用助听器干预,以改善听觉过敏、声源定位等症状为切入点,结合多项主、客观评估检测,适时调整助听器的调试方案,并全程辅助心理疏导,有效改善了患者的临床症状。此类隐性(不典型)听力损失患者诊断、干预较困难,没有可借鉴的临床案例,希望通过本案例对医生和听力师提供借鉴经验,以引发更多的重视和思考。

In this case, a patient with normal hearing threshold but symptoms of sensory hearing loss(decreased speech resolution in noisy situations,auditory hypersensitivity tinnitus, sound source localization disorders, etc.) was intervened by hearing aids. With the improvement of symptoms such as hypersensitivity and sound source localization as the starting point, combined with a number of subjective and objective assessment and detection, the debugging scheme of the hearing aid was adjusted in a timely manner, and the psychological counseling was assisted throughout the process, which effectively improved the clinical symptoms of the patient. For patients with such manifestations, some scholars have proposed the concept of hidden(atypical) hearing loss. Due to the difficulty of diagnosis and intervention(treatment, application of auxiliary equipment), there are no clinical cases to learn from. It is hoped that through this case, doctors and audiologists will provide experience that can be learned from, and arouse more attention and thinking.
