摘要
目的分析系统性红斑狼疮(SLE)合并真菌感染外周血Toll样受体9、抗黏病毒1(MX1)、趋化性细胞因子受体5(CCR5)表达及意义。方法选取2020年3月-2021年3月温州医科大学附属衢州医院(衢州市人民医院)收治的70例SLE合并真菌感染患者为研究组,选取同期60例医院收治的SLE患者为SLE组,另外选取60名健康体检者为健康组。采用实时荧光定量聚合酶链式反应(PCR)对三组外周血Toll样受体9、MX1表达进行检测,流式细胞仪检测CCR5表达。结果研究组Toll样受体9、MX1、CCR5相对于健康组和SLE组升高,差异具有统计学意义(P<0.05)。随着SLE活动性程度的加重,Toll样受体9、MX1、CCR5的升高加剧,重度活动、中度活动SLE患者Toll样受体9、MX1、CCR5水平高于轻度活动SLE患者(P<0.05)。受试者工作特征(ROC)曲线显示,与Toll样受体9、MX1、CCR5单项诊断相比,三项联合对SLE合并真菌感染的诊断价值较高(P<0.05)。结论Toll样受体9、MX1、CCR5表达升高对SLE合并真菌感染的发生起到了促进作用,临床上早期诊断及评估SLE合并真菌感染可通过以上指标进行。
OBJECTIVE To observe the expression of toll-like receptor 9,myxovirus resistance 1(MX1)and chemotactic cytokine receptor 5(CCR5)in systemic lupus erythematosus(SLE)patients complicated with fungal infection and analyze the significance.METHODS A total of 70 SLE patients complicated with fungal infection who were treated in Quzhou Hospital Affiliated to Wenzhou Medical University(Quzhou People’s Hospital)from Mar 2020 to Mar 2021 were assigned as the study group,meanwhile,60 SLE patients were chosen as the SLE group,and 60 healthy people who received physical examination were assigned as the healthy group.The expression levels of peripheral blood toll-like receptor 9 and MX1 were detected by means of real-time fluorescent quantitative polymerase chain reaction(RT-PCR),and the expression level of CCR5 was detected by flow cytometry.RESULTS The expression levels of Toll-like receptor 9,MX1 and CCR5 of the study group were significantly higher than those of the healthy group and the SLE group(P<0.05).With the aggravation of activity,the expression levels of toll-like receptor 9,MX1 and CCR5 were increasingly higher and higher;the expression levels of the SLE patients with severe activity and moderate activity were significantly higher than those of the SLE patients with mild activity(P<0.05).Reiver operating characteristic(ROC)curve showed that the value of the joint detection of Toll-like receptor 9,MX1 and CCR5 was significantly higher than that of the single detection in diagnosis of fungal infection in the SLE patients(P<0.05).CONCLUSION The elevated expression levels of Toll-like receptor 9,MX1 and CCR5 induce the occurrence of fungal infection in the SLE patients,and the above indexes can be used for early diagnosis of fungal infection in the SLE patients.
