摘要
目的探讨组织蛋白酶G(CTSG)基因多态性与创伤性骨髓炎发病风险的关联。方法选择2019年3月-2021年3月山东省东营市胜利油田中心医院骨科因四肢创伤入院治疗发生创伤性骨髓炎患者76例为研究组。选择同期医院收治的普通四肢骨折患者76例为对照组,对照组无任何感染性疾病。患者在入院后抽取外周静脉血采用实时荧光定量聚合酶链式反应(RT-PCR)法扩增GSTG基因rs45567233位点序列通过CT值判断基因分型。所有患者均在常规分离血清后检测血沉(ESR)、C-反应蛋白(CRP)和白细胞介素-6(IL-6)水平。结果研究组平均病程(16.15±2.58)周;研究组GSTG基因rs45567233携带AA基因型频率低于对照组,携带GG基因型和G等位基因频率高于对照组,差异具有统计学意义(P<0.05),两组携带AG基因型频率比较,无统计学差异;多元Logistic回归分析结果显示,GSTG基因rs45567233位点显性模型(GG+AG/AA)、隐性模型(GG/AG+AA)和等位基因模型(G/A)下发病风险均增加(P<0.05);创伤性骨髓炎患者GSTG基因rs45567233位点携带GG型血清ESR、CRP和IL-6水平高于AA型和AG型(P<0.05)。结论GSTG基因rs45567233位点携带GG基因型显著增加四肢创伤后感染型骨髓炎发病风险。
OBJECTIVE To explore the association between gene polymorphism of cathepsin G and risk of traumatic osteomyelitis.METHODS A total of 76 patients with traumatic osteomyelitis who were hospitalized in orthopedics department of Shengli Oilfield Central Hospital due to trauma of extremity from Mar 2019 to Mar 2021 were assigned as the study group,meanwhile,76 patients with common fractures of extremities were chosen as the control group.The peripheral venous blood specimens were collected,the GSTG gene was amplified by real-time polymerase chain reaction(RT-PCR),the genotypes at rs45567233 locus were identified by CT value.The levels of serum erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and interleukin-6(IL-6)of all the patients were detected.RESULTS The average course of disease of the study group was(16.15±2.58)weeks;the frequency of AA genotype at rs45567233 locus of GSTG gene of the study group was significantly lower than that of the control group,the frequencies of GG genotype and G allele of the study group were significantly higher than those of the control group(P<0.05),there was no significant difference in the frequency of AG genotype between the two groups.The result of multivariate logistic regression analysis showed that the dominant model(GG+AG/AA),recessive model(GG/AG+AA)and allele model(G/A)may increase the risk of disease(P<0.05).Among the patients with traumatic osteomyelitis,the levels of serum ESR,CRP and IL-6 of the patients who carried with GG genotype at rs45567233 locus of GSTG gene were significantly higher than those of the patients who carried with AA genotype and AG genotype(P<0.05).CONCLUSION The GG genotype at rs45567233 locus of GSTG gene may remarkably increase the risk of infectious osteomyelitis.
