味觉受体家族基因多态性与真菌性鼻-鼻窦炎易感性的关联

Association between polymorphism of taste receptor family gene and susceptibility to fungal rhinosinusitis

摘要 目的探讨味觉受体家族基因多态性与真菌性鼻-鼻窦炎易感性的关联。方法选择2017年1月-2021年4月杭州市余杭区第一人民医院耳鼻喉科收治的真菌性鼻-鼻窦炎患者60例为研究对象,同期医院因鼻息肉进行手术切除患者60例为对照组。采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)对苦味受体(TAS2R38)和甜味受体(TAS1R2)基因型、等位基因分布进行检测。结果研究组TAS2R38 AVI/AVI基因型、AVI等位基因频率高于对照组,差异具有统计学意义(P<0.05),两组TAS2R38 PAV/PAV、PAV/AVI基因型以及TAS1R2 TT、TG、GG基因型以及T、G等位基因比较,无统计学差异;以是否发生真菌性鼻-鼻窦炎为因变量,Logistic回归分析结果发现,携带AVI/AVI基因型以及AVI等位基因为真菌性鼻-鼻窦炎发病的危险因素(P<0.05);TAS2R38 PAV/PAV、PAV/AVI、AVI/AVI三组患者病原菌、合并鼻息肉、VAS评分以及受累鼻窦比较,无统计学差异,AVI/AVI患者Lund-kennedy评分高于其他两组,差异具有统计学意义(P<0.05)。结论苦味受体基因TAS2R38可能增加健康人群对真菌性鼻-鼻窦炎易感性,且与病情严重程度有关。 OBJECTIVE To explore the association between polymorphisms of taste receptor family gene and susceptibility to fungal rhinosinusitis.METHODS A total of 60 patients with fungal rhinosinusitis who were treated in otolaryngology department of the First People’s Hospital,Yuhang District,Hangzhou from Jan 2017 to Apr 2021 were recruited as the study subjects,meanwhile,60 patients who underwent surgical resection due to nasal polyps were assigned as the control group.The bitter taste receptor(TAS2 R38)and sweet receptor(TAS1 R2)genotypes as well as alleles were detected by using polymerase-chain-reaction-restriction-fragment-length polymorphism(PCR-RFLP).RESULTS The frequencies of TAS2 R38 AVI/AVI genotypes and AVI allele of the study group were higher than those of the control group,and there were significant differences(P<0.05).There were no significant differences in the TAS2 R38 PAV/PAV,PAV/AVI genotypes,TAS1 R2 TT,TG,GG genotypes and T,G alleles between the two groups.Set the occurrence of fungal rhinosinusitis as the dependent variable,the result of logistic regression analysis showed that the AVI/AVI genotype and AVI allele were the risk factors for the fungal rhinosinusitis(P<0.05).There were no significant differences in the pathogens,complication with nasal polyps,VAS score and involved sinuses among the three groups of patients with TAS2 R38 PAV/PAV,PAV/AVI and AVI/AVI;the Lund-kennedy score of the AVI/AVI patients was higher than that of other two groups,and there was significant difference(P<0.05).CONCLUSION The bitter taste receptor gene TAS2 R38 may increase the susceptibility to fungal rhinosinusitis among healthy population and is associated with the severity of disease.
