超声在肺术后围手术期肺不张检测中的临床新进展 被引量:1

摘要 胸外科患者进行肺部手术治疗后容易出现各种并发症,围手术期肺不张是导致胸外科患者术后并发症发生的关键因素,还会延长患者的住院时间,提高致残率和病死率。因此,需从肺不张检测及肺保护通气等方面入手,以减少或避免胸外科患者肺术后发生肺不张。肺部超声是一种安全、可行、无创的床旁成像方式,且易于使用,便于对患者的病变情况进行动态的观察,在肺部疾病诊断中应用广泛,也可用于围手术期肺不张的诊断,同时超声可通过可视化指导肺保护性通气,对肺复张进行检测,指导选择最佳的呼气末正压,并结合患者的个体化资料,实施个体化的肺保护通气策略,从而有助于减少肺术后患者肺不张的发生。为了减少肺术后围手术期肺不张的发生,本文综述了超声在肺术后围手术期肺不张检测中的应用。 Thoracic surgery patients are prone to various complications after lung surgery.Perioperative atelectasis is a key factor leading to postoperative complications in thoracic surgery patients.It also prolongs the patient’s hospital stay and increases disability.Rate and fatality rate.Therefore,it is necessary to start with atelectasis detection and lung protection ventilation to reduce or avoid atelectasis after lung surgery in thoracic surgery patients.Lung ultrasound is a safe,feasible and non-invasive bedside imaging method,and it is easy to use,which is convenient for dynamic observation of the patient’s pathological changes.It is widely used in the diagnosis of lung diseases and can also be used for perioperative atelectasis.At the same time,ultrasound can visually guide lung protective ventilation,detect lung recruitment,guide the selection of the best positive end-expiratory pressure,and implement individualized lung protective ventilation strategies based on the patient’s individualized data.Help reduce the occurrence of atelectasis in patients after lung surgery.In order to reduce the occurrence of atelectasis in the perioperative period after lung surgery,this article reviews the application of ultrasound in the detection of atelectasis in the perioperative period after lung surgery.
