2021年我院门诊抗菌药物使用情况分析

Analysis of the use of antimicrobial drug in outpatient clinics in our hospital in 2021

目的 调查本院门诊抗菌药物的使用情况,了解其使用特点,发现存在的问题并提出改进措施,旨在进一步提高抗菌药物合理使用水平。方法 由本院处方点评系统调取2021年1月1日~12月31日门诊药品使用数据,统计门诊使用的抗菌药物类别、品种、剂型、规格、数量、金额、使用人次、联合用药、使用分级、使用途径等。分析各类抗菌药物剂型、使用分级构成情况、用药频度(DDDs)、金额及其排序;门诊各科抗菌药物药占比、使用率及联合用药情况;各科抗菌药物分级及剂型使用情况。结果 调查期间本院门诊使用抗菌药物涉及8类24种,使用金额2221867.34元。其中,口服剂型涉及15个品种,使用金额2147863.47元,占抗菌药物使用金额的96.67%,针剂16个品种,使用金额81275.41元,占抗菌药物使用金额的3.66%。非限制抗菌药物使用金额2175572.2元,占抗菌药物使用金额的97.92%,限制性使用抗菌药物使用金额46295.14元,占抗菌药物使用金额的2.08%,无特殊使用级抗菌药物的使用。门诊药品使用金额为54623991.96元,抗菌药物使用金额2221867.34元,占药品使用金额占比为4.07%。总用药人次275028人次,使用抗菌药物43078人次,使用率15.66%。抗菌药物单用39769人次,占比为92.32%,二联、三联使用分别3273、47人次,占比分别为7.60%、0.11%。未有抗菌药物四联使用情况。门诊使用非限制级抗菌药物使用率为97.11%,限制使用级抗菌药物使用率为3.77%。门诊抗菌药物口服剂型使用率为97.99%,针剂使用率为2.14%。结论 经过多年综合治理,本院门诊抗菌药物使用率大幅度降低,用药合理性大为提高,但仍存在不合理使用现象,须进一步加强监管。

Objective To investigate the use of antimicrobial drug in outpatient clinics, understand the characteristics of their use, find out the existing problems and propose improvement measures to further improve the level of rational use of antimicrobial drug. Methods The outpatient drug use data from January 1 to December 31, 2021 were retrieved from the hospital prescription review system, and the categories, varieties, dosage forms,specifications, quantities, amounts, number of users, co-medication, use classification, and routes of use of antimicrobial drug used in outpatient clinics were counted. The antimicrobial drug dosage form, use classification composition, defined daily dose system(DDDs), amount and its ranking;proportion, usage rate and combined use of antimicrobial drug in outpatient departments;classification of antimicrobial drug and usage of dosage forms in each department were analyzed. Results The outpatient use of antimicrobial drugs in our hospital during the investigation period involved 8 categories and 24 kinds, with the an amount of 2221867.34 yuan. Among them,oral dosage forms involved 15 varieties, with an amount of 2147863.47 yuan, accounting for 96.67% of the amount of antimicrobial drug used, and 16 varieties of injections, with an amount of 81275.41 yuan, accounting for 3.66%of the amount of antimicrobial drug. The amount of unrestricted use of antimicrobial drug was 2175572.2 yuan,accounting for 97.92% of the amount of antimicrobial drug used. The amount of restricted use of antimicrobial drug was 46295.14 yuan, accounting for 2.08% of the amount of antimicrobial drug used. There was no use of special-use antimicrobial drug. The amount of outpatient drug use was 54623991.96 yuan, and the amount of antimicrobial drug use was 2221867.34 yuan, accounting for 4.07% of the amount of drug used. The total number of drug use was 275028 person-times, and the use of antimicrobial drugs was 43078 person-times, with a utilization rate of 15.66%. Antimicrobial drugs were used alone for 39769 person-times, accounting for 92.32%,and dual-combination and triple-combination drugs were used for 3273 and 47 person-times, accounting for 7.60% and 0.11%, respectively. There were no cases of quadruple use of antimicrobial drugs. The utilization rate of non-restricted antimicrobial drugs for outpatient use was 97.11%, and the utilization rate of restricteduse antimicrobial drugs was 3.77%. The utilization rate of outpatient antimicrobial drugs in oral dosage form was 97.99%, and the utilization rate of injections was 2.14%. Conclusion After years of comprehensive management,the use rate of outpatient antimicrobial drug has been significantly reduced and the rationality of drug use has been greatly improved, but the phenomenon of unreasonable use still exists and supervision must be further strengthened.
