自拟祛痛逐瘀汤配合毫针治疗膨隆型腰椎间盘突出症效果观察

Clincal effect of self-made Qutong Zhuyu Decoction combined with filigree needles in the treatment of bulging lumbar disc herniation

目的观察自拟祛痛逐瘀汤配合毫针治疗膨隆型腰椎间盘突出症的效果。方法选取2018年12月至2020年12月海口市中医医院收治的82例膨隆型腰椎间盘突出症患者为研究对象,采用随机数表法分为对照组和观察组,每组41例。对照组患者应用单一毫针治疗,观察组患者则应用自拟祛痛逐瘀汤配合毫针治疗,疗程均15 d。比较疗程结束后两组患者的治疗效果、不同时间视觉模拟疼痛(VAS)评分以及腰椎功能、日常活动能力和生活质量。结果观察组患者的治疗总有效率为95.12%,明显高于对照组的70.73%,差异有统计学意义(P<0.05);治疗前,两组患者的VAS评分比较差异无统计学意义(P>0.05);治疗后7 d和15 d,观察组患者的VAS评分分别为(3.22±1.06)分、(1.02±0.93)分,明显低于对照组的(5.68±1.24)分、(4.28±1.19)分,差异均有统计学意义(P<0.05);治疗前,两组患者的生理功能、精神状态、社会功能、情感职能评分比较差异均无统计学意义(P>0.05);治疗后两组患者的上述四项评分均大幅度提高,且观察组评分明显高于对照组,差异均有统计学意义(P<0.05);治疗前,两组患者的腰椎功能、日常活动能力比较差异均无统计学意义(P>0.05);治疗后,观察组患者的腰椎功能、日常活动能力明显优于对照组,差异均有统计学意义(P<0.05)。结论自拟祛痛逐瘀汤配合毫针治疗膨隆型腰椎间盘突出症可缓解患者疼痛,改善机体腰椎功能与日常活动能力,并提高患者生活质量,临床应用效果显著。

Objective To observe the effect of self-made Qutong Zhuyu Decoction combined with filigree needles in the treatment of bulging lumbar disc herniation.Methods A total of 82 patients with bulging lumbar intervertebral disc herniation who were admitted to Haikou Hospital of Traditional Chinese Medicine from December 2018 to December 2020 were selected as the research objects,and divided into control group and observation group by random number table method,with 41 patients in each group.The patients in the control group were treated with a single filiform needle,while the patients in the observation group were treated with the self-made Qutong Zhuyu Decoction combined with filigree needle.The course of treatment was 15 d.The treatment effect,Visual Analogue Scale(VAS)score at different time,lumbar spine function,daily activity ability,and quality of life were compared between the two groups after treatment.Results The total effective rate of the observation group was 95.12%,which was significantly higher than 70.73%of the control group(P<0.05).Before treatment,there was no significant difference in the VAS scores between the two groups(P>0.05);7 d and 15 d after treatment,the VAS scores of the observation group were(3.22±1.06)points and(1.02±0.93)points,respectively,which were significantly lower than(5.68±1.24)points and(4.28±1.19)points in the control group(P<0.05).Before treatment,there was no significant difference in the scores of physiological function,mental state,social function,and emotional function between the two groups(P>0.05);after treatment,the above four scores of the two groups were significantly improved,and the scores in the observation group were significantly higher than those in the control group(P<0.05).Before treatment,there was no significant difference in lumbar spine function and daily activities between the two groups(P>0.05);after treatment,the lumbar spine function and daily activities of the observation group were significantly better than those of the control group(P<0.05).Conclusion Self-made Qutong Zhuyu Decoction combined with filigree needles in the treatment of bulging lumbar intervertebral disc herniation can relieve pain,improve lumbar spine function and daily activities,and improve the quality of life of patients,with remarkable clinical application effect.
