不同年龄段体检人群腰椎椎旁肌退变与骨盆-脊柱参数的相关性

Correlation Between Lumbar Paraspinal Muscle Degeneration and Pelvic-Spine Parameters in Physical Examination Population of Different Age Groups

目的:探究不同年龄段体检人群腰椎椎旁肌退变与骨盆-脊柱参数相关性。方法:回顾性分析在2020年1月-2022年2月于揭阳市人民医院接受腰椎正侧位X线检查及MRI检查的192例体检者临床资料,统计分析不同年龄段体检者腰椎椎旁肌退变(椎旁肌CSA/椎体CSA)与骨盆-脊柱参数[骶骨倾斜角(SS)、骨盆倾斜角(PT)、腰椎前凸角(LL)]之间相关性。结果:L_(3/4)、L_(4/5)、L_(5)/S_(1)的椎旁肌CSA/椎体CSA水平,在<40岁、40~49岁、50~59岁、≥60岁体检者中逐渐下降,差异有统计学意义(P<0.05)。<40岁、40~49岁、50~59岁、≥60岁体检者中,L_(3/4)、L_(4/5)、L_(5)/S_(1)的椎旁肌CSA/椎体CSA值与SS、LL呈正相关,差异有统计学意义(P<0.05);与PT无相关性,差异无统计学意义(P>0.05)。结论:随着年龄增长,腰椎椎旁肌退变程度逐渐增加,并出现明显骨盆-脊柱相关参数变化,而在年龄≥60岁体检者中,其腰椎椎旁肌退变越严重,其骨盆-脊柱参数中SS、LL正相关,应用价值高。

Objective:To explore the correlation between lumbar paraspinal muscle degeneration and pelvic-spine parameters in physical examination population of different age groups.Methods:From January 2020 to February 2022,the clinical data of 192 physical examinees who received lumbar spine anterioposterior and lateral X-ray examination and MRI examination in Jieyang People′s Hospital were retrospectively analyzed.Correlation between lumbar paraspinal muscle degeneration(paraspinal muscle CSA/vertebral CSA)and pelvic-spine parameters[sacral slope(SS),pelvic tilt(PT),lumbar lordosis(LL)]was statistically analyzed.Results:Among the physical examinees aged<40 years,40~49 years,50~59 years and≥60 years,the CSA values of paravertebral muscle/vertebral body in L_(3/4),L_(4/5) and L_(5)/S_(1) were gradually decreased,and the difference was statistically significant(P<0.05).Among the physical examinees aged<40 years,40~49 years,50~59 years and≥60 years,the CSA values of paravertebral muscle/vertebral body in L_(3/4),L_(4/5) and L_(5)/S_(1) were positively correlated with SS and LL,and the difference was statistically significant(P<0.05);they were not correlated with PT,and the difference was not statistically significant(P>0.05).Conclusion:With the increase of age,the degree of lumbar paraspinal muscle degeneration gradually increases,and there are obvious changes in pelvic-spine related parameters.Among the physical examinees aged≥60 years,the lumbar paraspinal muscle degeneration and pelvic-spine parameters SS,LL are positively correlated,and has high application value.
