精神分裂症患者暴力行为危险因素分析

Risk factors of violent behavior in patients with schizophrenia

目的 探讨精神分裂症患者暴力行为的危险因素。方法 回顾性分析2018年1月至2020年6月收治的501例精神分裂症患者,进行基线资料调查,并评估患者当前危险度分级,根据患者是否符合暴力行为标准分为两组,即无暴力行为组(n=386),存在暴力行为组(n=115),同时经二元Logistic回归分析影响因素。结果 经二元Logistic回归分析,年龄<60岁(OR=112.565)、情绪易激动(OR=195.227)、存在攻击行为史(OR=1784.830)、首次发病年龄≤30岁(OR=148.232)、间断服药或不服药(OR=27.665)、家庭月收入<5000元/月(OR=180.202)是导致患者出现暴力行为的独立因素。监护人属于可疑因素。结论 首次发病年龄、服药情况、年龄、家庭月收入、攻击行为史、情绪是导致患者出现暴力行为的独立因素,应对相应危险因素进行干预及预防。

Objective To explore the influencing factors of violent behavior in patients with schizophrenia. Methods A retrospective analysis of 501 patients with schizophrenia were admitted between January 2018 and June 2020, baseline data surveys were conducted, and the current risk grades of the patients were assessed. The patients were divided into two groups according to whether they met the criteria for violent behavior.Namely the non-violent behavior group(n=386 cases), the violent behavior group(n=115), and the influencing factors were analyzed by binary logistic regression. Results After binary logistic regression analysis, age<60 years old(OR=112.565), emotional irritability(OR=195.227), history of aggressive behavior(OR=1784.830), age of first onset ≤30 years old(OR=148.232), intermittent medication or no medication(OR=27.665), family monthly income <5000 yuan/month(OR=180.202) were independent factors that lead to violent behavior in patients. The guardian was a suspicious factor. Conclusion Age of first onset, medication status, age, family monthly income, history of aggressive behavior, and emotion are independent factors that lead to violent behavior in patients. Corresponding risk factors should be intervened and prevented.
