等离子电切术治疗前列腺囊肿对患者术后恢复及国际前列腺症状评分的影响

Effect of transurethral bipolar plasma kinetic resection prostatectomy for prostate cyst on postoperative recovery and international prostate symptom score

目的 探讨等离子电切术(TUPKRP)治疗前列腺囊肿对患者术后恢复及国际前列腺症状评分表(IPSS)评分的影响。方法 回顾性分析2018年1月至2020年1月本院泌尿外科收治的70例前列腺囊肿患者的临床资料,根据治疗方式不同分为对照组(n=34)与观察组(n=36)。对照组采用前列腺电切术(TURP)治疗,观察组采用TUPKRP治疗。比较两组围术期指标、国际前列腺症状评分表(IPSS)评分、尿流动力学指标及术后并发症发生情况。结果 观察组手术时间、导管留置时间及住院时间均短于对照组,手术总出血量少于对照组,囊肿切除质量多于对照组,差异有统计学意义(P<0.05)。术后3个月,两组IPSS评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。术后3个月,两组膀胱压力、最大尿流率(Qmax)均高于治疗前,残余尿量(RUV)均少于治疗前,且观察组膀胱压力、Qmax均高于对照组,RUV少于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为11.11%,低于对照组的38.24%,差异有统计学意义(P<0.05)。结论 采用TUPKRP治疗前列腺囊肿效果确切,可缩短患者手术时间,缓解前列腺囊肿症状,改善尿流动力学指标,减少并发症发生率,利于改善患者预后。

Objective To investigate the effect of transurethral bipolar plasma kinetic resection prostatectomy(TUPKRP) for prostate cyst on postoperative recovery and international prostate symptom score(IPSS) score.Methods The clinical data of 70 patients with prostate cysts admitted to the Department of Urology of our hospital from January 2018 to January 2020 were retrospectively analyzed,and they were divided into control group(n=34) and observation group(n=36) according to different treatment methods.The control group was treated with transurethral resection prostatectomy(TURP),and the observation group was treated with TUPKRP.The perioperative indicators,International prostate symptom scale(IPSS) score,urodynamic indicators and postoperative complications were compared between the two groups.Results The operation time,catheter indwelling time and hospitalization time of the observation group were shorter than those of the control group,the total blood loss during the operation was less than that of the control group,and the cyst removal quality was higher than that of the control group,and the differences were statistically significant(P<0.05).At 3 months after operation,the IPSS scores of the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).At 3 months after operation,bladder pressure and maximum urine flow rate(Qmax) in the two groups were higher than those before treatment,and residual urine volume(RUV) was less than before treatment,the bladder pressure and Qmax in the observation group were higher than those in the control group,and the RUV was less than that in the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 11.11%,which was lower than 38.24% in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment effect of TUPKRP in patients with prostate cyst is definite,which can shorten the operation time,relieve the symptoms of prostate cyst,improve the urodynamic indexes,reduce the incidence of complications and improve the prognosis of patients.
